Objective To investigate the relationship between the serum concentrations of adiponectin, interlekin-18(IL-18), C-reactive protein(CRP) and the insulin resistance in the patients with type 2 diabetes mellitus(T2DM). To observe the effects of metformin on the serum levels of adiponectin , IL-18 and CRP in overweight/obese type 2 diabetes and the anti-inflammatory effect of metformin.Methods Based on the body mass index(BMI), 60 cases of type 2 diabetes were divided into two groups: (1) overweight/obese T2DM group( group DM1): 30 cases with BMI≥23kg/m~2; (2) non-obese T2DM group(group DM2): 30 patients with BMI between 18.5 and 22.9 kg/m~2. 30 overweight/obese and non-obese persons each was chosen as the control group 1(group C1) and control group 2(group C2). Their sex, age, BMI, waist circumference and waist to hip ratio(WHR) were matched with group DM1 and DM2 respectively. The body height, weight, blood pressure, waist and hip circumference, adiponectin, IL-18, CRP, fasting plasma glucose(FPG), fasting serum insulin(FINS) and blood-lipid were measured, and then BMI and homoeostasis model assessment(HOMA-IR) were calculated in all subjects. The insulin resistance index was evaluated by HOMA-IR. According to the value of HOMA-IR, the T2DM and control groups were separated into two sub-groups respectively in order to observe the differences of the inflammatory factors in the different degree of IR . The changes of the above indexes were observed before, four weeks and eight weeks after the treatment of metformin in 20 overweight/obese patients with T2DM.Results (1) T2DM group had significantly higher value of HOMA-IR (0.50±0.24 vs -0.03±0.32), lower concentration of adiponectin (1.30±0.80 vs 5.97±2.14 mg/L)and higher...
