Exploration Of The Cause For Negative Result Detected Of HBVDNA-positive Patientes'HBeAg

HBV DNA is present regarded as the gold standard by which doctors judge wheather hepatitis B virus is replicating or not.Positive HBV DNA indicate that hepatitis B virus is replicating.Hepatitis B virus DNA>10~3 copy /ml is judged that HBV DNA is positive.Hepatitis B virus has four editting genes:S gene,P gene,C gene and X gene. Pre S1 gene in S gene edite Pre S1 protein. Pre S1 protein is better marker by which doctors know the replicating condition of hepatitis B virus, and highly relative with HBV DNA . HBeAg was taked as the marker for knowing replicating condition and infectivity of HBV befor,but many factors will lead to detect the negative HBeAg of the HBV in the replicating condition by ELISA.Therefor,there is the phenomeno that the negative HBeAg of the hepatitis B virus with postive HBV DNA and Pre S1 protein is detected by ELISA.76 sepcimens with negative HBeAg, positive HBV DNA and PreS1 protein were collected and annalzed respectively from Postzone hook effect, HBeAg/IC , the gene mutation in BCP and the gene mutation in Pre C region of HBV to explain the obove phenomeno in this thesis.Studies showed that : (1) five cases diluted in the 76 specimens were detected HBeAg by ELISA and the positive rate was 6.6%.(5 cases in the HBeAg-/ HBeAb-model,and the positiv rate is 8.2%;0 cases in the HBeAg-/ HBeAb + model, the positiv rate is 0%.). the study conclusion showed that:the five cases existed Postzone hook effect ,and it's HBeAg could not be detected by ELISA for the HBeAg density greatly higher than coated antibody.(2)38 cases in the 76 specimens were detected for formed HBeAg/IC.the positive rate was 50%.(30cases in the HBcAb+ / HBeAb-model .positive rate is 49%;8cases in the fifteen specimens with HBcAb+ / HBeAb+ model, positive rate is 53%).then,this conclusion indicated that the 38 cases could be detected positive HBeAg by routin ELISA for HBeAg/IC.(3) HBeAg of 24 cases could not be expressed for the gene mutation in Pre C region of HBV and was 31.58% in the all study specimen. so,there was not HBeAg in the blood of 24 HBV carrier.
