Objective: The affected spine in patients with ankylosing spondilitis(AS) is irreversible. Early diagnosis is the key to obtain a favorable prognosis, but the common diagnostics standard for AS is unable to early diagnosis. Most of the undifferentiated spondylarthropathy (uSpA) will have developed to AS, thus intensive studies on spondylarthropathy (SpA), specially follow-upon uSpA are of important value. By this study the clinical features and developing regularity of SpA and uSpA in natural population would be found out, thereby it may enhance clinical doctor's diagnostic and therapeutic levels and provide the preventing and treating measures on rheumatic diseases.Methods: Using modified national-international anti-rheumatic disease union survey schedule, according to survey procedure of community project of control of rheumatic diseases(COPCORD) of WHO, namely the patients with stagesⅠandⅡwere investigated by health care staff on each family one by one, patients with stageⅢand positive answer were examined by the rheumatologist, and susceptible patients withⅣstage received the related X-ray and laboratory examination. Finally, disease classification and summery were performed. HLA-B27 and rheumatoid factor(RF) were tested on all susceptible patients, and in first week polvis nomotopoc and lumbar nomotopic/alterl X-ray films were taken, which were read by two radiologists under ignorance of illness state. Finally, according to Amor standard the diagnosis was made by 3 rheumatologists. All the data were input in a computer, then analyzed statistically with SPSS for Windows. The prevalence rate was standardized using the Taiyuan city population of nationwide census in year 2000 as standard group.Results: Actually investigated number of residents aged≥16 years were 3915 accounted for 97.75% of the investigated people. The number of people with rheumatic symptom were 996(25.44%) and finding rate of stageⅢwas 90.76%(904/996). Standardized prevalence rates of rheumatic symptoms were 15.23% for male and 20.89% for female. Symptom frequency on in different sites was higher in female than in male except hip joint. The most common symptoms were knee pain, lumbago and cervicodyma, and standardized prevalence rates were 9.77%, 8.45% and 4.97% respectively. Of 42 patients, 8 showed HLA-B27 positive and all showed RF-negatively. Of 328 patients with lumbar film, 218 showed lumbar osteoarthritis, 3 showed bamboo joint-like change, which all were male. Of 58 patient with pelvis film, 2 were with gradⅠsacroiliitis, 12 with gradeⅡ, 3 with gradeⅢand 1 with gradeⅣ.
