| BackgroundCoronary heart disease(CHD) is one of important disease endangering human-being's health. The incidence and mortality rate of CHD are gradually increase in China recently. With technology progress, the internal medicine treatments of CHD have developed rapidly, Especially appearance of percutaneous coronary intervention (PCI), it removed stenosis of coronary artery, is the effectivest therapy to Myocardial Ischemia.But PCI has many deficient. Studys found the recanalization of great artery isn't equal to good perfusion. Myocardial microcirculation is disorder of therapeutic effect of PCI. When opacification display TIMI 3, there is 25-30% of myocardium couldn't be perfusedl. The factors impact myocardial microcirculation is hemodynamics, the function of vascular endothelial cell, Inflammatory reaction, corpuscular apoptosis. And these factors lead ischemical reperfusion injury, no-reflow, and so on. The proliferation of vascular smooth muscle cells(VSMC) is a key pathological event of artherosclerosis(AS) and restenosis(RS) after revascularization.Utilizing the multitarget of Chinese Traditional Medicine (TCM), decrease the complication of PCI, increase the result of PCI, expand directions of integrate medicine of TCM. Tongguan Capsule have significant result to CHD and patients after PCI. Much studies found Tongguan Capsule can improve hemorheology, blood clotting, platelet aggregation.This article study Improved effect of Tongguan Capsule to myocardial effective perfusion after PCI. Study observe improved effect of stethalgia, cardio electrophysiology and ventricular recon-stitution. Then the article study relation QT dispersion(QTd) to the types of CHD and coronary artery. ObjectiveObserve the tongguan capsule effect to clinic symptom, electrophysiology and ventricular reconstitution of CHD after PCI. Discuss the mechanism of effect myocardial reperfusion after PCI and the improve of Tongguan Capsule. To observe the effect of Tongguan Capsule on proliferation and of vascular smooth muscle cells(VSMC)of rabbits by serum pharmacological method in vitor.MethodUsing prospective Randomized, Controlled study, we observed 60 CHD patients who treated by PCI in Guangdong provincial hospital from Oct 2005 to March 2006. According to Randomized table we drive the patients to the trial group (Tongguan Capsule + western medicine) and control group(westem medicine). On the day before PCI and in the month afer PCI we observe the CHD patients in TCM symptoms, P dispersion(Pd), QTd/QTcd, the degree of ST-segment recovery, ventricular wall dyskinasia index, and BNP. The data putted into computer and statisticed by SPSS11.0. ResultRespectively compared patients' indexes correlation on clinic syndromes, trial group and control group have been obviously improved, and trial group better than control group, which exists notable statistics sense(p<0. 01s￡p<0.05). Compared the stethalgia, trial group is better than control group, which exists notable statistics sense(p<0. 01). Pd and QTd/QTcd of trial group than control group, which exists notable statistics sense(p<0.01§!cp<0.05). Trail group's degree of ST-segment recovery is better than which of control group. Trail group's improve of ventricular wall dyskinasia index and BNP is better than control group, which exists notable statistics sense(p<0.01§￡p<0.05). There were significant difference in QTd/QTcd among different type of CHD and the affection of coronary artery, which exists notable statistics sense(p <0.01). The MTT value of the Tongguan Capsule group is smaller than control group, the difference between two groups is marked, P<0.05. Cone I us i onTongguan capsule can improve the clinic syndromes and the effect myocardial reperfusion of CHD after PCI. Tongguan Capsule is no only fit to QDCUXUEYU types of patients with CHD, but also to QIXUTANYU types. QTd/QTcd are associated with clinical phenotypes of coronary heart disease. Tongguan Capsule could markedly inhibit the proliferation of VSMC.
