Objective: To analyze the relationship between body fat content and fat distribution in type 2 diabetes mellitus and free fatty acids and insulin secretion.Methods: Included were 94 cases of type 2 diabetics, divided into two subsets according to body mass index or waist circumference :obese and non-obese group(BMI≥25); waistline augment and normo-waistline (male≥90cm or female≥80cm).To measure convention inspection item of clinical, plasma glucose, serum insulin and free fatty acids concentrations. Body fat content was measured by Bioelectric Impedance Analvsis. Index of insulin resistance and insulin secretion were calculated by Homeostasis model, and early phase of insulin secretion was reflected by ?INS30/?BG30. Glucose clamp technique was performed in 30 cases of objects, to determine exactly the function of insulin secretion and to calculate total volume dose of First-phase of insulin secretion, mean of Second-phase of insulin secretion, mean of 120~150min of insulin secretion, peak value of First-phase of insulin secretion, peak value of Second-phase of insulin secretion, increment of First-phase of insulin secretion and increment of Second-phase of insulin secretion.Results: (1) The level of plasma free fatty acids in obesity group of type 2 diabetics were significantly higher than that in non-obesity group (p<0.05);(2) The level of plasma free fatty acids in waistline augment group of type 2 diabetics were significantly higher than that in normo-waistline group (p<0.05);(3) Compared with non-obesity group of type 2 diabetics, increment of First-phase of insulin secretion in obesity group of type 2 diabetics decreased(P<0.05); Other indexes of glucose clamp test and index of early phase...
