Objective: To investigate the effect of supplementation with Alanyl-glutamine dipeptide on insulin resistance and outcome in critically ill COPD and respiratory failure patients.Methods: A prospective, randomized and controlled study. All patients who were admitted to HUAXI hospital ICU between Jan 2005 and Feb 2006 were selected and randomized into two groups. Two groups were given the similitude nutrition support protocol. Treatment group: nonprotein calorie was 25kcal/kg · d,50% of its calorie was provided by fat emulsion, the nitrogen according to each 0.2g/kg to supply, among them,20%-25% of nitrogen was given from the parenteral nutrition by the alanyl-glutamine dipeptide, the rest uses the equilibrium ammo acids.
