Object By analyzing the difference of image grade of CT and grade of clincal diagnosis in infants with hypoxic ischemic encephalopathy disscuss the cause of the difference .By measure of CT values of white matter evaluate its value for diagnosis of HIE. To evaluate the value of CT in diagnosis and prognosis on infants with HIE.Method we had examined 74 term infants with HIE using CT in 10days after birth,and compared with clinical grade .Then we compared CT images of 35 normal infants and 44 slight HIE infants and measured values of white matter in theirs.10 HIE infants CT and MRI images were observed relatively .Result There was concordance among CT image grade and clinical grade ,but the concordance is low( Kappa statistic, P<0.05,K<0.75). We mixed CT images of 35 normal infants and 44 slight HIE infants. Double-blinded observion found ,in slight HIE infants CT scan had 61.4% detetion rate.In normal infants, accuracy of rate was 80% and false positive is 20%.The mean value of white matter in nomal infants was 22.4±3.3Hu, and mean value of HIE was17.1±2.1Hu. Although their mean value had difference,the distribution of value overlapped. In 10 HIE infants that CT and MRI scan,there were 8 abnormalities in CT images,in which local edema were 4 cases, generally edema were 4 cases , hyperintersity in basal ganglia 1 case,SAH were 3 cases. There were 10 abnormalities in MRI images,in which local edema were 6 cases, generally edema were 4 cases , hyperintensity in deep and subbortical white matter(TWI) was 4 cases.Conclusion The coincident rate between image grade of CT and grade of clincal diagnosis in infants is low-rise.HIE infants clinical diagnosis are important than CT. CT scan is a support method. Early CT images can show brain injured ,such as edema and hemorrhage. Grade of CT image is not absolute standard of prognosis in HIE infants. MRI scan is better than CT in HIE infants'diagnosis .Preterm HIE infants'diagnosis is different with full infants.So we should more careful in preterm infants'diagnosis.
