PurposeVentricular remodeling after myocardial infarction (Ventricular remodeling,VR) refers to the emergence of the change of complex molecular signals and the re-expression of embryonic gene and protein in the heart when the heart is under damaged and overloaded condition after an acute myocardial infarction. This result in myocardial cells’ maladaptation of hypertrophy,myocardial apoptosis and myocardial extracellular matrix fibrosis, clinically manifested as ventricular hypertrophy,increased ventricular volume and ventricular structural changes. The further development of ventricular remodeling after myocardial infarction can lead to heart failure,sudden death. It is one of the most important risk factors for future cardiogenic death.The clinical and laboratory experiment confirmed the effect of using Chinese traditional medicine to combat Ventricular remodeling is reliable and proved the traditional Chinese medicines have favorable prospect in treatment of ventricular remodeling. Taohong Siwu Decoction which come from (YiZongJinJian)) is compose of Peach kernel, Safflower, Chinses Angelica, White peony root, Radix Rehmanniae Preparata, Rhizome. Modern medical research confirmed that there are many effects of Taohong Siwu Decoction, such as anti-inflammatory, Relieving pain, dilated vessels, improving microcirculation and increasing non-specific immunity.ect.There are empirical studies and clinical applications showed that the Taohong Siwu Decoction can improve myocardial ischemia, increase coronary flows, and expand the constrictive coronary vessels, butthere was not enough research on ventricular remodeling after acute myocardial infarction. But the modern pharmacology mechanism of treatmenting chronic heart failure is still not so clear, In this experimental study, The experiments were carried out about the effects on the indexes of echocardiographic evaluations、left ventricle mass index and and heart mass index、pathologically observed through HE and Masson trichrome staining technic、the protein of myocardial tissue collagen Ⅰ and collagen Ⅲ through immunohistochemical methods、the express of ATi of CHF rats to discuss its function mechanism initially, Thus to further enrich and improve the treatment theory of ventricular remodeling and to provide new research ideas and methods for standarlizing the treatment of ventricular remodeling by Traditional Chinese Medicine.MethodSelecting male Wistar rats SPF level,ligating the left coronary artery and resulting in the myocardial infarction. Dividing those falling into the VR standard into 6 groups:Model control group,Taohong Siwu Decoction group with high dosage, medium dosage, low dosage, Captopril group; plus Sham operation group. 12 in each group,a total of 72. Taohong Siwu Decoction with high dosage:9.72g/kg-d herb medicine decoction administered orally; Taohong Siwu Decoction with medium dosage:4.86g/kg-d herb medicine decoction administered orally; Taohong Siwu Decoction with low dosage:2.43g/kg-d herb medicine decoction administered orally Captopril group:1g/L captopril suspension with free drinking water;Sham operation group and Model control groups:normal drinking water. Continuous administration for 12 weeks. Using Doppler echocardiography to checkout on 12 week after operation.12 weeks later the rats were killed, the heart mass index(HMI) was calculated and the myocardial tissues were sampled and then pathologically observed through HE staining technic and Masson trichrome staining technic. Testing the protein of myocardial tissue collagen Ⅰ,collagen Ⅲ through immunohistochemical methods; observing the effect of Taohong Siwu Decoction on the express of AT1 of rats after myocardial infarction by Western Blot.Statistical analysis:SPSS 13.0 statistical soft was used. Statistical analysis Results were expressed as mean ± S.D.Statistical significance was determined using one-way ANOVA, multiple comparisons using Fisher’s LSD post hoc tests. The differences were considered statistically significant at a value of P< 0.05.Result1 Results of cardiac ultrasonographyCompared with the sham group, model group rats had significantly increased LVDd、LVDs values and declined EF、FS values (P<0.001),Taohong Siwu Decoction roup with high dosage, medium dosage, low dosagegand Captopril group can had significantly declined LVDd、LVDs values and increased EF、FS values (P<0.01)compared with the model group. but there was no significant difference between Taohong Siwu Decoction group and Captopril group. Taohong Siwu Decoction with high dosage is thel best of the three dosage of Taohong Siwu Decoction.2 The Result of heart quality/body constitutions in experiment ratsCompared with the sham group, model group rats had significantly increased heart weight and HMI significantly(P<0.01); Compared with the model group, heart weight and HMI decreased significantly in Taohong Siwu Decoction group with high dosage, medium dosage, low dosage and Captopril group rats, the difference was significant (P<0.01), but there was no significant difference between them(P> 0.05). and Taohong Siwu Decoction with high dosage is thel best of the three dosage of Taohong Siwu Decoction.3 Result of HE stainingSham operation group:the pigmentation of myocardium is even and in neat band, the cell boundary is clear, indicating normal myocardial morphology. Model comparison group:thickening, elongation, disordered arrangement and widened interspace of myocardial fibers, the cell nucleus is big and dark stained indicating the infiltration of inflammatory cells. The junctional zone of Taohong Siwu Decoction group with high dosage, medium dosage, low dosage and Captopril group shows mild myocardial cell hypertrophy and hyperplasia with spotted fibroplasias, clear and neat myocardial cells display around the non infarct zone, less inflamed cell infiltration compared with the Model control group, and Taohong Siwu Decoction with high dosage is thel best of the three dosage of Taohong Siwu Decoction.4 Result of Masson trichrome stainingMyocardium was stained in brown, collagen fibers in blue under the light microscope. Sham operation group:no cardiac muscle collagen fibroplasias, myocardial cells are neatly displayed,distinct light and dark shade,normal morphology; Model control group:rat’s myocardial myofibril gap is significantly widened,disorganized, with hypertrophy of myocardial cells,visible focal myofilament dissolving, large number of collagen fibers appear in both infarct areas and non-infarct regions, coarse collagen fibers connected into a network, disorganized, and unevenly distributed. aohong Siwu Decoction group with high dosage, medium dosage, low dosage and Captopril group:rats’ myocardial myofibril and myofilament are in good order, normal morphology, evenly and thinly distributed collagenous fibers, myocardial mesenchyme fibrosis obviously decreased compared with the Model comparison group, but the occurrence is still inevitable.5 Expression results of Immunohistochemical detection of myocardial collagen type Ⅰ,Ⅲ collagen proteinSham operation group:no significant expression of collagen,no stromal fibrosis;Model control group:obvious fibrosis,showing significant expression of collagen; Taohong Siwu Decoction group with high dosage, medium dosage, low dosage and Captopril group:a small amount of collagen fiber, obviously much less than that in Model control group, but the occurrence is still inevitable. The content of myocardium collagen and the ratio of myocardial collagen type Ⅰ to Ⅲ collagen protein in model control group were obviously higher than the sham operation group(P<0.01); the content of myocardium collagen and the ratio of myocardial collagen type Ⅰ to Ⅲ collagen protein in Taohong Siwu Decoction group with high dosaee. medium dosage, low dosage and Caotopril group were lower than which in model control group(P<0.01); the ratio of myocardial collagen type Ⅰ to Ⅲ collagen protein in Taohong Siwu Decoction group with high dosage, medium dosage, low dosageand Captopril group was insignificant (P> 0.05). and Taohong Siwu Decoction with high dosage is thel best of the three dosage of Taohong Siwu Decoction.6 Effect of Taohong Siwu Decoction on AT1 in myocardial infarction ratsCompared with the sham group, the rate of AT1 increased in model group rats(P<0.01). Taohong Siwu Decoction group and Captopril group can inhibit the express of AT1 in myocardial infarction rats (P<0.01), but there is no significant difference between them(P> 0.05).Conclusion1、Rat with acute myocardial infarction can develop myocardial hypertrophy, decreased heart function, myocardial tissue fibrosis, myocardial apoptosis after 12 weeks of conventional feeding,those rats can serve as a model for studying left ventricular remodeling.2、The treatments used in Taohong Siwu Decoction can improve cardiac function of the rat,reduce myocardial fibrosissmaintain myocardial cells’ normal morphology,inhibit the express of AT1 in myocardial infarction rats same effect as using Captopril.3、Taohong Siwu Decoction might be one of Effective medicine in traditional Chinese medicine treatment for anti-ventricular remodeling after myocardial infarction.
