Objective To estimate the efficacy of prophylactic platelet transfusion in the preoperative preparation of cirrhotic portal hypertension patients .Methods We performed a retrospective review of 163 patients of cirrhotic portal hypertension from January 2000 to January 2005. First,divided the patients into two groups based on the phenomenon of with or without surgery related hemorrhage and then picked out the risk factors of hemorrhage by analysing the preoperative indexes.Second, evaluation of the efficacy of prophylactic platelet transfusion by the assessment of clinical and laboratory data . Results 1.The preoperative liver function classification and the platelet count both keep a key role in the surgery related hemorrhage, and the liver function classification is the most important factor . 2.Though with the decline of platelet count the danger of hemorrhage is increasing,the platelet count cannot be used as the only factor of predicting whether hemorrhage will happen or not .3. The efficacy of prophylactic platelet transfusion is closely related with preoperative liver function classification , there is no significient difference between the prophylactic platelet transfusion group and the non-transfusion group in the patients of Child-pugh classification A of liver function;but the difference is very distinctive between the transfusion group and the non-transfusion group in the patients of Child-pugh classification B or C.Conclusions The prophylactic platelet transfusion can effectively decrease the danger of surgery related hemorrhage in patients of cirrhotic portal hypertension,but the suitable patients must be strictly selected to ensure not to take the liberty of using it .
