Objective To investigate theories of liver injury,protective effects of ulinastatin (UTI) on liver and give the clinical proof for the use of ulinastatin in cardiopulmonary bypass during open heart surgery.Methods A total of thirty patients under cardiopulmonary bypass during open heart surgery were randomly divided into two groups (group test or group A n=15, group control or group B n=15 ).The patients in group A receive ulinastatin(UTI 15000UI/kg) and those in group B not receive it. Blood samplase were withdrawn from central vein for the measurements of tumor necrosis factor-a(TNF-a),interleukin-8(IL-8) and interleukin-6(IL-6),alanine aminotrans- ferase(ALT) and total bilirubin(TB) before the operation,30 min after aortic and cardiac cannulation,1h and 6h after the aortic declamping and 24h after the operation by radioimmunity and automatic biochemistry analysator.Results In group A,the leves of TNF-a,IL-8,IL-6,ALT and TB were lower than that in group B at each time during and after the operation. Conclusion UTL can protect liver function in cardiopulmonary bypass during open heart surgery.
