Objective :Type 1 diabetes is an autoimmune disease characterized by T-cell mediated autoimmune destruction of pancreatic P-cells. It's pathogeny due to a combination of genetic and environmental factors . .Infection of Coxsackie B virus is one of the major risk factors. Immune mechanism plays a main role in the occurring of type 1 diabetes. The aims of this study were to examine the CVB-IgM (CVB3-IgM, CVB4-IgM , CVB5-IgM ) in sera and cell factors including interleukin-1β, interleukin-12, interleukin-18, tumor necrosis factors-a in sera with Type 1 diabetes, and to investigate the relation between infection of Coxsackievirus group B and type 1 diabetes with Th1/Th2 imbalance. Content:Type 1 diabetes was diagnosed based on the standard of WHO.The CVB-IgM (CVB3-IgM, CVB4-IgM, CVB5-IgM)in sera and cell factors including.Interleukin -1 β, interleukin-12, interleukin-18, tumor necrosis factors-α in sera with 40 patients( 22 males, 18 females ) and 40 normal controls were detected by ELISA. Results:1 .CVB-IgM was detected in 42.5% of the patients and in 7.5% of normal control groupP<0.05.The proportion of CVB3~IgM , CVB4-IgM , CVB5-IgM in sera are 11.8 % , 64.7 % ,and 23.5% respectively .2. The levels of IL-lp, IL-12 > IL-18, TNF-a in patient were higher than that in controls .Both IL-12 and IL-ip were significantly higher as compared with two groups. (P< 0.01)3. The levels ofIL-1p^ IL-12, IL-18, TNF-a in Type 1 diabetic patients with positive CVB — IgM were higher than those with negative . Plasma IL-1P > IL-18 and TNF-a were significantly higher as compared with two groups.4. Plasma IL-ip> IL-12, IL-18-. TNF-a in seven patients were detected in pretherapy and post-treatment by insulin . The leves of IL-ip, IL-12n IL-18> TNF-a in post-treatment group were lower than that of pretherapy group . Plasma IL-ip were significantly lower than that of pretherapy group .(PO.01)Conclusion :The results suggested that Type 1 diabetes and infection of Coxsackievirus group B was correlated. Virus-mediated autoimmune disorder makes T-cell activation. The increase of IL-18 promote IL-ip> IL-12 and TNF-a secretion . Cell factors activateseveral pathways of apoptosis and promote the P-cell apoptosis .CVB infection is associated with T-cell mediated immunity in Type 1 diabetic patients.
