Objective To study the probability and reliability of laparoscopic splenectomy (LS). Methods The clinical data of 35 cases who underwent laparoscopic splenectomy since June 1999 were analyzed retrospectively, including 29 cases of idiopathic thrombocytopenic purpura (ITP) and 6 case of cirrhosis with splenosis. Results 33 cases of LS were successfully performed. 22 cases took dorsal desubitus position, their mean operative time was 3.5 hours and mean blood loss was 150ml. The operative time of the case who took right lateral desubitus position was 4.0 hours and the blood loss was 80ml. The mean operative time of 10 cases who took right inclined lateral desubitus position was 2.5 hours and the mean blood loss was 100ml. 1 patient was underwent novo-laparotomy in 6 hours after LS because of intraabdominal hemorrhage. 2 cases were required conversion to open splenectomy during the operation. Conclusions LS has the characters of minimally invasion, few blood loss, quick recovery and short admitting time. And the complications of LS are preventible and can be reduced to a degree as low as possible. LS is a safe, effective and feasible therapy method.
