Objective: Liver transplantation is the last treatment of various advanced liver disease. Acute rejection (AR) is the most common complication and reason for postoperative mortality after liver transplantation.Rat is the better experimental animal in transplantation research. Refer to related reports, we plan to establish this modified model of liver transplantation which is stable and simple and feasible. Up to now, diagnosis of AR is based on a combination of clinical signs, liver function tests, and core needle biopsy (CNB) of hepatic graft. It is deficiency of specificity and sensitivity. The purposes of this study were to determine the value of detecting of the concentrations of IFN-γ in peripheral blood for prediction and diagnosis of AR.Methods: First, study the skills for building model of ROLT in wistar rats performed by the two cuff method. And then, rats were randomly divided into two groups (n=15/group): Allogeneic group SD-wistar ROLT group;and syngeneic group Wistar-Wistar ROLT. The survival time were analyzed, and the clinical characters, the liver function, the transplantated liver pathologic character and the concentrations of IFN-γ in serum was observed and measured in postoperative penod.The relationship was made between concentrations of IFN-γ in allogeneic group and syngeneic group and AR following liver transplantation in rat by statistics.Results: 37 cases of ROLT were performed in the second period,the operativesuccess rate was 91.9%,The mean survival time of allogeneic group were 25 days, while the wistar-wistar recipients could lived for long time without any rejection.The liver function recovered well in syngeneic while in allogeneic group we found the liver function recovered slowly and the evident AR in pathologic result. The concentrations of IFN-y had marked difference ( P< 0.01) in peripheral blood between the allogeneic group and syngeneic group.Conclusion: On the basis of cuff technique,we anastomosed the portal vein and subhepatic vena cava with cuff and connected the suprahepatic vena cava to the recipient's disphragm with 7-0 running suture. This model is stable and can be used in the experiment of liver transplantation. We could find the evident AR character in SD-Wistar rats, and in this group the concentrations of IFN-y were marked high. IFN-y could be a index to estimate the AR reaction.
