Clinical Study On Serum IGF-Ⅱ In Type 2 Diabetic Nephropathy

ObjectiveTo investigate serum IGF-Ⅱ concentration in Type 2 Diabetes and in controls, espeically in diabetic nephropathy patients.And futher explore the relationship between IGF- II and blood glucose,insulin,C-peptide,insulin resistance, β-cell function,lipid and other clinical points.Design and MethordsAccording to the latest Type 2 Diabetes diagnostic and typing standard formulated by WHO,we studied 178 probands those were divided into three groups.30 were in control group(group0),including 15 males and 15 females,from 32 to 64 years old.148 were T2DM patients,including 80 males and 68 females,from 28 to 88 years old. In T2DM patients,there were non-diabetic nephropathy group(group1) (95 individuals) and diabetic nephropathy group(group2) (53 individuals).And in group 2,there were two subsets—micro- albuminuria group and macroalbuminuria group.The fasting IGF-Ⅱ concentration of these 178 individuals were measured by ELLSA kit which was bought from American RapidBio company.And body height,body weight,body mass index(BMI),systolic blood pressure(SBP),diastolic pressure(DBP) waistline and lipid were measured;in T2DM patients,we also measured HbA1c,fasting blood glucose(FBG), postprandial blood glucose(PBG),fasting insulin(FINS), postprandialinsulin(PINS) , fasting C-peptide(FCP) > postprandial C-peptide(PCP),growth hormone(GH)> creatinine(CrK urea nitrogen(BUN;K 24-hour urine protein > 24-hour urinary albumin(UAE), creatinine clearance rate(Ca% homeostasis model assessment insulin resistance index(HOMA-IRI).All the data were into the computer,analyzed by SPSS software system.Results1. the general clinical points among group 1 > group 2 and group 0:(1) To the three groups, the patients of group 1 and 2 were older than group 0;and there was no difference among the three groups about the sex,height,weight,BMI, waist-to-hipratio,DBP,TG,TC,LDL-c,VLDL-c,HDL-c,uric acid,Cr,BUN.To the Ccr, the group 1 and 2 increased significantly in contrast with the group 0.(2) There was also no difference between the group 1 and group 2 in FBG,PBG,FCP,PCP, FINS, PINS,HbAlc.2. the concentration of IGF- II in the three groups( X ± s):The serum IGF- II concentration of group 1 (76.07 ± 64.00 ug/1) was significantly lower than that of group 0 (102.45 + 40.73 ug/1) (P<0.05). And no difference was found between group 0 (102.45 + 40.73 ug/1) and group 2 (91.75± 74.79 ug/1) .There was also no difference between group 1 and group 2.3. the correlation between the IGF- II and other clinical points(1) Analysis of correlation revealed that there was no significant relationship between IGF- II and GH(P<0.05),and that also happened to IGF- II and HOMA-IRI.(2) The correlation analysis showed that the UAE had significant positive correlation with IGF-II(P
