Objective: To investigate the superiority and probability of the surgical treatment of spondylolisthesis with the combined use of a self-designed device consisting of the reinforced ring and pedicular nail, and to provide the theory for spondylolisthesis with this apparatus.Methods: 30 cases of spondylolisthesis were treated with the combined use of a self-designed device consisting of the reinforced ring and pedicular nail. One side breakage of interarticularis in 19 cases, both sides in 11 cases. The lesions were of the â…  degree in 18 cases , â…¡ degree in 10 cases and â…¢ degree in 2 cases. Take posterior approach under euthyphoria with positor, put two spiral marking nails into olisthe centrum and reinforced ring on the vertebral plate, combined the spiral marking nail and reinforced ring with a suspender, then tight screw nut of the spiral marking nails to recover antedisplacement of olisthe centrum. Through the posterior approach , bone graft was done around the facets and the transverse processes.Results The follow-up period extended for more than half oneyear , all 18 cases I degree lesions achieve anatomical reposition;5 cases in II degree lesions achieve anatomical reposition and others achieve fundamental reposition;1 case achieve partial reposition and the other unsat reposition in 2 cases III degree lesion . The symptoms disappeared and no recurrence in 28 cases I and II degree lesions, anatomical reduction was attained in 90.3% of the cases with healing of the bone graft in all .Conclusion The application of this method avoidedintra-operative loosening of the pedicle nails;the number of pedicle nails used could be reduced;the sites of nails insertion were selected without the restrictions by instrumentation;The operation and lying in bed period was shortened;reliable fixation and high reduction rate can be obtained;the operation were easy to manipulate and generalizate.
