To evaluate the effect of schistosomiasis control, understand the trend and the general schistosomiasis morbidity changes in Hubei province, this thesis analyse the overall shistosomiasis control situation and varied subtypes status in endemic areas based on the data from the random sampling with stratifying and clustering method in 1989,1995 and 2001 in Hubei province, so as to provide the scientific basis for making the control program for the relevant department. The analyzed data included resident infection rate and infection intensity with different sex, age and occupation as well as buffalo infection rate and so on. Meanwhile, to promote the work efficiency, the method of blood sample absorbed on filter paper was probed into the epidemiological survey in the fourth sampling survey of Hubei province in 2004. The results are briefly introduced as below.1 . Analysis of overall morbidity changes in three sampling surveys of schistosomiasis in Hubei provinceSurveyed data was based on the three sampling surveys from 1989~2001 in schistosomiasis endemic areas of Hubei province. The results showed that the infection rate of schistosomiasis in population decreased, while the EPG in patients and population gone up in the three sampling surveys (1989, 1995, 2001). On the sex aspect, the infection rate and EPG of mankind was higher than that of female, while the infection intensity of female gone back much more than that of mankind. On the age aspect, the highest infection rate was in the age group of 40-50. On the occupation aspect, the infection rate and EPG in farmers, fishermen and boatmen were higher than the preschool, businessmen and other occupation population. As the buffalo infection, it showed a decreasing tendency. The result indicated that although current control strategies has obtained obvious effects on the morbidity control of schistosomiasis through integrated measures, the EPG in patients and population gone up, which showsinfection and reinfection situation are still serious in Hubei province..2. Analysis of morbidity changes in three sampling surveys of schistosomiasis from endemic areas with different subtypes in Hubei provinceThere are three varied subtypes of endemic areas of schistosomiasis in Hubei province: flood plain, inner dike in lake and marshland of endemic area and hill area. Analysis of morbidity changes of different subtypes were carried out in the three sampling surveys. The resident infection rate and EPG were the highest in flood plain. On the sex aspect, the infection rate and EPG of mankind was higher than that of female in flood plain area and inner dike area, while it was on the contrary in hill area. On the age aspect, the infection rate and EPG of population over 30~ age group were higher than that of below 30~ age group. On the occupation aspect, in 2001, the farmer's infection rate and EPG were the highest in flood plain area and hill area, and boatman in inner dike area. The highest infection rate of buffalo was in inner dike in 1989, in flood plain area in 1995 and in hilling area in 2001. The morbidity changes of schistosomiasis in different subtype endemic areas showed significant difference. So the special and effective integrated measures should be selected according to the local situation to strengthen and consolidate the achievement of schistosomiasis.3. Study of whole blood sample absorbed on filter paper for detecting antibodies of schistosomiasis in the field.To assess the practical value of ELISA and IHA using dried blood filter paper absorbed micro amount of plasma of peripheral blood from finger tip for schistosomiasis examination in the filed, the study of a whole blood sample absorbed on filter paper (DBFP) was tested as a substitute for serum. Serum samples were obtained from subjects aged from 6 to 60 years from two villages with high schistosomiasis prevalence by venipuncture and finger-prick from the same individual. ELISA and IHA using serum and dried blood filter paper were compared. At the same time, a compare of cost and compliableness for the examination using the two methods was carried out. The results showed that there was no significant difference between the two methods,the cost including material, labor power. Cost of using filter paper was lower than that of using serum, while the compliableness of using filter paper was higher than that of using serum. It was suitable for dried blood filter paper application in the field due to its easy performance, high detection rate, lower cost and high compliableness.
