Systemic lupus erythematosus(SLE) is a high morbidity rate in women's period of duration,and its pathogenesis is not clear completely. SLE is caused by the coagulation of some factors, such as, heredity, virus infection, some environments and hormones, which can change the cell structures of individual's tissues, or can cause the mutation of the immunity activity cells. As a result, the body loses the immunological tolerance of itself and causes the disorder of immunological regulation.SLE is a autoimmune disease involving any organ of body, and has a few of clinical manifestations. It is so variable and atypical in clinical manifestations of the blood system that it is easy to misdiagnose. According clinical experience, when the patients of SLE have the decrease of platelet count (PLT), the symptoms are often more serious than SLE with the normality of PLT, and the therapy is more difficult than the latter. There are many reasons which can cause the decrease of PLT in SLE. In so many autoimmune antibodys produced by SLE, platelet antibody and cardiolipin antibody have the nearest relationship with the decrease of PLT.judging the patient's conditions of SLE, we often use systemic lupus erythematosus disease activity index(SLEDAI) to evaluate them. SLEDAI ,which has a high objectivity ,is regarded as the golden standard of measuring the activity of the patients of SLE. But, its content of the standard of perfection, which has 24 indicators, is so much that it has some limits in clinical works.With making progress in technology and proving in study instruments ,especially, with flow cytometry (FCM) entered into the clinical studies , we have gone up a high step in studying SLE. We can have a general comprehension for diagnosis, therapy and therapeutic effectiveness of SLE through using it, which mainly assays lymphocyte subgroups and their functions. For its rapid, sensitiveness and accuracy, at same time, FCM is widely used in the diagnosis and therapy of a few of diseases through measuring platelet activation. Nowadays, however, there is few in studying platelet activation of the patients of SLE using FCM.This study is a tentative study for the platelet changes in SLE. It includes two parts: the first part is belong to the investigation of epidemiology which analyse the clinical characters of between the patients of SLE having the decrease of PLT and having the normality of PLT; the second part is experimentation part which studies the expression of CD62P and CD63 in patients with systemic lupus erythematosus and their correlations with SLEDAI.Aims: l.To explore the clinical and laboratory features of systemic lupus erythematosus (SLE) with the decrease of PLT.2. To observe the expression of CD62P and CD63 of the peripheral blood in patients with systemic lupus erythematosus, and their correlation with SLEDAI. Methods: 1.34 cases of SLE with the decrease of PLT were reported. Their first sign and clinical manifestations, involvement of organs and laboratory findings were analysed and compared with another 40 cases of SLE with the normality of PLT who were elected at random.2. To measure CD62P,CD63 expression of 10 cases of systemic lupus erythematosus using flow cytometry(FCM) and compare with those of 20 cases of healthy volunteers, meanwhile, calculate the score of SLEDAI of 10 cases of SLE. Results: 1. The incidence of the involvement of kidney was more than that with the normality of PLT, but the incidence of butterfly ererythema and the arthritis were less than the latter, there were significant difference statistically. The incidence of decrease of some laboratory examinations, such as CH50, WBC and the innormality of urine protein, and the other incidence of incease ,such as PCT(thrombocytocrit) and PDW (platelet distribution width) were more than that with the normality of PLT.2. The expression of CD62P and CD63 were significantly higher than the healthy volunteers(P<0.01).CD62P was positively correlated with SLEDAI(P<0.05). Conclusion : l.The clinical manifestasions and Laboratory examinations in SLE with the decrease of PLT have differences to some extent compared with SLE with the normality of PLT.2.The expression of CD62P and CD63 of the peripheral blood were significantly highly in patients with systemic lupus erythematosus.3.CD62P can be regarded as a indicator that can estimate the activity of systemic lupus erythematosus.
