Background and objective:The incidence of esophageal carcinoma is high and the mortality of esophageal carcinoma is situated on the fourth in our country. It threatens seriously people's health. The cancerous occurrence and development in esophageal are due to many factors participation and many stages evolution, the most cases that caused the death of esophageal carcinoma are its invasion and metastasis. So the study about pathogenesis, development, invasion and metastasis of esophageal carcinoma has become a focus.Recently, the researches on the relation between IGF-1R, u-PA and the tumor are increasing constantly. A great deal of studies show that the high expression of IGF-1R lies in malignant nervous system tumor, liver cell carcinoma , breast carcinoma ,adrenalcarcinoma and lung carcinoma ,IGF-1R plays a important role in the malignant transformation , invasion and metastasis. There are some studies show that u-PA takes part in the pathogenesis and development of ovary carcinoma, carcinoma of stomach, carcinoma of the prostate, carcinoma of large intestine, etc. Furthermore, it has something with the invasion and metastasis of the tumors. About the expression of IGF-1R in esophageal carcinoma and correlation between IGF-1R and the expression of u-PA haven't been reported. To explore the role that IGF-1R and u-PA play in pathogenesis, development, invasion, metastasis of esophageal carcinoma and the correlation between IGF-1R and the expression of u-PA. This research subject investigates, through immunohistichemistry SP method, the expressions of IGF-1R protein and u-PA protein in the tissue of esophageal carcinoma, in the tissue adjacent to cancer and in the tissue of normal esophageal, so as to discusses its mechanism of pathogenesis, development, invasion and metastasis, to find an useful indicator for early diagnosis of esophageal carcinoma and provide the new methods and ideas for therapy and predict the prognosis of esophageal carcinomaMaterials and methods:By adopting immunohistichemistry SP method, the expressions of IGF-1R protein and u-PA protein in the tissue of 54 normal esophageal cases, 22 adjacent to cancer cases and 54 esophageal carcinoma cases (21 cases accompany with the lymph node metastasis) were detected. All the datas were analyzed by SPSS 11.0 statistical software. The comparison of positive rates uses the Chi-square and Fisher, Statistically significant level was considered as "alpha equals 0.05".Results:1. The rates of positive expression of IGF-1R protein in the tissue of esophageal carcinoma, in the tissue adjacent to cancer and in the tissue of normal esophageal are 92.59%. 59.09% and 20.37% respectively, the difference between any two group is significant (P<0.01); The rates of powerful positive expression of IGF-1R protein in thetissue of esophageal carcinoma , in the tissue adjacent to cancer and in the tissue of normal esophageal are 76.00%, 46.15% and 9.09% respectively, the difference between any two group is significant(P<0.05 or P<0.01).2. The rates of positive expression of u-PA protein in the tissue of esophageal carcinoma, in the tissue adjacent to cancer and in the tissue of normal esophageal are 74.07% > 45.45% and 14.82% respectively, the difference between any two group is significant (P<0.01); The rates of powerful positive expression of u-PA protein in the tissue of esophageal carcinoma , in the tissue adjacent to cancer and in the tissue of normal esophageal are 82.50%, 50.00% and 0.00% respectively, the difference between any two group is significant(P<0.05 ).3. In the tissues of esophageal carcinoma, the rates of powerful positive expression of IGF-1R protein on the low-muscle invaded, deep-muscle invaded and outer lay invaded are 28.75% ^ 65.00% and 90.48% respectively. The difference among the three groups is significant (P<0.01)4. The rates of powerful positive expressions of IGF-1R protein in the tissue of non-metastasis and metastasis are 51.72% and 95.24% respectively. The difference in the two groups has statistical significance (P < 0.01).5. In the tissues of esophageal carcinoma, the rates of powerful positive expressions of u-PA protein on the low-muscle invaded, deep-muscle invaded and outer lay invaded are 37.50% ^ 80.00% and 95.24% respectively. The difference among the three groups was significant (P<0.01).6.The rates of powerful positive expressions of u-PA protein in the tissue of non-metastasis and metastasis are 63.64% and 90.48% respectively. The difference in the two groups has statistical significance (P < 0.05).7. There are 54 positive expressions of u-PA protein cases in the 74 positive expressions of IGF-1R protein cases. There are 51 negative expressions of u-PA protein cases in the 56 negative expressions of IGF-1R protein cases. The IGF-1R expressionwas closely correlated with the u-PA expression (P < 0.01) Conclusions:1. The rates of positive expressions of IGF-IR protein and u-PA protein are relatively low in the tissue of normal esophageal, but, in proper order, it obviously increases in the tissue adjacent to cancer and in the tissue of esophageal carcinoma. It is pointed out that IGF-IR and u-PA take part in the pathogenesis and development of esophageal carcinoma. IGF-IR and u-PA are expected to be the biological index of illness state examination and initial diagnosis.2.The powerful positive expressions of IGF-IR protein and u-PA protein obviously concern the invasion of esophageal carcinoma and lymph node metastasis. It indicated that the detection of IGF-IR and u-PA in the tissue of esophageal carcinoma might be helpful to estimate the invasion, lymphatic metastasis and prognosis of esophageal carcinoma.3. The IGF-IR expression was closely correlated with the u-PA expression,which suggested that IGF-IR and u-PA played a positive role to each other in course of pathogenesis, development, invasion and metastasis of esophageal carcinoma, the detection of the IGF-IR expression combined with u-PA expression might may have more significant meaning to explore the mechanism of pathogenesis, development, invasion and metastasis of esophageal carcinoma and to judge predict the prognosis of esophageal carcinoma.
