The primary liver cancer is the one of the mostcommon malignant tumors in China, especially in southeastarea of our country. The median age of patients with livercancer in China is 40-50 and it is common in man.Recently its incidence of liver cancer has increased. Onthe basis statistic of ministry of health in 1995, themortality rate of liver cancer in China has the secondhighest mortality rate. The key of improving the effect oftreatment are early diagnose, early therapy and multipletherapy. The major type of primary liver cancer in China ishepatic cell cancer(91.5%). The primary liver cancercan easily invade the embranchment of portal vein,embolism of tumor move through portal vein engendersbroadcast among the liver, even blocking the stem of portalvein. In the recent years there are studies showing thatMMP-2 has the closed relation with the metastasis of thetumor. It plays the important role on the invasion andmetastasis of the tumor. TIMP-4 is the inhibitor of theMMP-2. So we plan to study the expression of theTIMP-4,MMP-2 among the patients with liver cancer.Objective:We did immunohistochemistry analysis onthe expre-ssions of TIMP-4,MMP-2 in 15 nomal and 40malignant tumor specimens respectively by TIMP-4monoclonal antiboby and MMP-2 monoclonal antibody.Atthe same time we did correlative analysis on some agentsthat affect the prognosis of liver cancer.Thus we try todiscover the role that TIMP-4,MMP-2 play in the onset andevolution of primary liver cancer and the relationshipbetween TIMP-4,MMP-2 and the infiltration and metastasisof liver cancer,and finally find out thesensitive ,convient,practical methods for early diagnosisand prognosis. Method:We analyzed the expression ofTIMP-4,MMP-2 in 15 samples with normal commonliver organize, and 40 samples of liver cancer byimmuneohistochemisty.The 15 normal common liverorganize specimens come from forensic medical departmentof jilin university,and the 40 slides specimens are fromresection specimens performed in the second hospital ofjilin university during April 1998 to December2004,which were finally diagnosed by anatomy and allwithout chemotherapy preoperation.Statisticalmethod:the difference of each marker among normal liverorganize and malignant organize and the differentstages of liver cancer were analysed by Wilcoxonmethod,and the correlation relationship among the 2markers and influences of them to the prognosis of livercancer were analyzed by Spearman method (γs is thecorrelation coefficient). Results:1.Immunohistochemistry:⑴Expression ofTIMP-4:buffy grannles distributed in nuclearmembrane,cell membraneand and cytoplasm,mainly incytoplasm.Also cen been seen in normal and benignlesions,but very weak.⑵Expressions of MMP-2:darkimmunohistochemistry staining in benign duct tissue ,cancer cell and extracelluar matrix,but without normaltissue.2.The result shows that the expression ofTIMP-4,MMP-2 in primary liver cancer are obviouslyhigher than in normal( P< 0.01) ,their positive rate are80%,100% respectively.3.⑴The expression of MMP-2 inlow differentiation group is obviously higher than in highdifferentiation group ( P < 0.05 ); The expression ofTIMP-4 in high differentiation group is obviously higherthan in low differentiation group( P< 0.05).⑵The more thecell transfer degree the higher positive rates of MMP-2( P< 0.05) . The less the cell transfer degree the higherpositive rates of TIMP-4( P< 0.05) . Conclusion:The expression of TIMP-4,MMP-9 in theprimary liver cancer are higher than in normal,and theyhave positive correlation to multifactors,such asdifferentiation, transfer,etc. So we hope we can detectserum MMP-2,TIMP-4 later to make an earlydiagnosis.And the development of modern RT-PCRprovides us a new method.
