Objectives: To investigate the effect of ulinastatin on cytokines and its clinical performance on patients with acute respiratory distress syndrome (ARDS).Methods: In this prospective random clinical trail (RCT), twenty-one patients with ARDS were randomized into two groups: ulinastatin group (n =7) were administered with ulinastatin in addition to the conventional therapy; and non-ulinastatin group (n =14) underwent the conventional therapy. During the study, serum cytokines including tumor necrosis factor-a (TNF-α), interleukin-8 (IL-8), and interleukin-10 (IL-10) were measured, and the parameters of respiration and circulation were monitored. Results: There was a decreasing tendency in cytokines after the therapy in ulinastatin group. In non-ulinastatin group, an increasing tendency in cytokines was observed after the therapy. In ulinastatin group, the oxygenation index after the therapy were significant higher than those before the therapy (P < 0.05). In both groups, the respiratory rate was significantly reduced after the therapy (P < 0.05). Mortality and the incidence of multipleorgan failure (MOF) in ulinastatin group were significantly lowerthan those in non-ulinastatin group.Conclusions: Ulinastatin can inhibit the releasing of cytokinesin patients with ARDS. In addition, it can improve theoxygenation index and the respiratory function. The additionaladministration of ulinastatin on the basis of the conventionaltherapy will exert a better therapeutic effect on patients withARDS.
