Objectives: To observe apoptosis and the expression of bcl-2, bax protein in the peri -hematomal brain region of Intracerebral Hemorrhage(ICH) patients; so as to investigate into the relationship among neural apoptosis, the expression of Bcl-2, Bax pro -tein and Sthenia ZHENG, Protration ZHENG, as well as the relationship among apoptosis, hematoma volume and the scores of nervous impairment, to inquir the eff -ect of apoptosis in delayed neural injury following ICH and then to provide the mole -cular biology standard for the microcosmic syndrome differentiation with TCM to quest for new treatment.Materials and methods: 29 patients with ICH were collected and divided into three groups: YIN sthenia group, YANG sthenia group and protration ZHENG group. All were accord with the diagnosing point of ICH and must be able to be diagnosed as one of the syndromes based on the differentiation standards of TCM and undergone surgical operation, among which 16 were male and 13 were female. The average age was 65years(aged 42-78 years). 6 normal brain tissue were taken as normal control group .To collect the perihematomal brain tissue to take as pathological specimens through the evacuation intracerebral hematoma surgical parthway in ICH group. The standard of nervous impairment of acute cerebral accident was used to score, the DU -OTIAN formula to calculate the hemorrhage volume, the apoptosis rate were examin -ed with TUNEL (Terminal deoxynucleotidyl transferase mediated dUTP-nick-end -labeling) method and the bcl-2,bax gene expression was detected with Immuno-histochemisty method. Results:1. The apoptosis rate and the expression rate of bcl-2> bax protein in the perihemato -mal brain region of ICH patients significantly increased in comparison with the norm -al control groups (PO.01).2. The expression rate of bcl-2 protein showed obvious negative correlation to the apoptosis rate (P<0.01)and the expression rate of bax protein showed obvious positive correlation to the apoptosis rate in perihematomal brain region of ICH patients (P< 0.01). the Bax/Bcl-2 ration showed obvious positive correlation to the apoptosis rate in perihematomal brain region of ICH patients (P<0.01).3. The expression rate of bcl-2 protein showed obvious negative correlation to the hematoma volume (PO.01) and the expression rate of bax protein showed obvious positive correlation to the hematoma volume in perihematomal brain region of ICH patients (PO.01). the Bax/Bcl-2 ration showed obvious positive correlation to the hematoma volume in perihematomal brain region of ICH patients (PO.01).4. The apoptosis rate and the expression rate of bcl-2 > bax protein , Bax/Bcl-2 ration were significantly different among YIN stheniaN YANG sthenia and the protr -ation ZHENGs (PO.01, PO.05), The apoptosis rate of protration ZHENG was the highest, YANG sthenia second, YIN sthenia last; the expression rate of bcl-2 protein of YIN sthenia was the highest and protration ZHENG the lowest; The expression rate of bax protein of protration ZHENG was the highest, YIN sthenia the lowest. Bax/ Bcl-2 ration of protration ZHENG was the highest, YIN sthenia the lowest.5. The course of diseases were not obviously different among YIN stheniaN YANG sthenia and the protration ZHENGs (P>0.05). The hematoma volume and The scores of clinical nervous impairment were significant different among the three groups in ICH(PO.01). The volume and the scores of protration was the first, YANG sthenia second, YIN sthenia last.6. There were positive correlation among the apoptosis rate, hematoma volume and the scores of clinical nervous impairment(PO.Ol). There were no correlation amongThe course of diseases and that three indexes. Conclusion:1. The mechanism of apoptosis may be involved in the delayed brain injury after ICH in human.Gene Bcl^ Bax s Bax/Bcl-2 ration may play an important role during the period of apoptosis in ICH.2. Both the sthenia and protration ZHENGs in ICH could reflect the degree of brain impairment. The apoptosis rate and the expression rate of bcl-2> bax protein were obviously different among YIN sthenia> YANG sthenia and the protration ZHENGs. The apoptosis rate and the expression rate of bax protein -, Bax/Bcl-2 ration increased successively; the expression rate of bcl-2 protein was descended successively. The delayed brain injury of stroke prostration is more serious than stroke excess. The apo -ptosis may induce the severity of the state.3. The apoptosis rate showed obvious positive correlation to hematoma volume and the scores of clinical nervous impairment, the more volume,the more severity and the more apoptosis.4. The apoptosis rate and the expression rate of bcl-2> bax protein in the perihemato -mal brain region of ICH patients could serve as a microcosmic index to judge the diff -erence between stroke excess and stroke prostration.5. According to the study we attempt to treat ICH with medication of suppressing apoptosis to improve the ICH patient curative effect.
