PURPOSE : To assess the diagnostic values of 1H-MRS in differentiation benign from malignant soft tissue tumors. MATERIALS AND METHODS:22Patients with soft tissue tumors and 10lower limbs of normal muscles were studied in 1H-MRS.All patients were confirmed by surgical.and pathology.All patients were scanned with GE Singa 1.5T MR/I by knee coil ,flexible coil and head coil.All patients were underwent T1WI and T2WI sequences before 1H-MRS.Region of interest was included areas of the tumor and ROI was aovided inclusion of the adjacent bone cortex,adipose tissue and gas. After obtained the original images ,we did some analysis with the functool 2 software.Comparision of metabolites and metabolic ration including of Choline(Cho),Creatine(Cr),N-Acety1Aspartate(NAA) ,Lipids(Lip) ,lactic acid(Lac) ,Cho/Cr ,Cho/NAAand Cr/NAA were done among benign tumors,malignant tumors and normal muscles .The Metabolites and metabolic ration obtained from the benign tumors and malignant tumors were compared by using the one-way ANOVA Variance Analysis in 11.5 SPSS software to statistics all parameters. All results were described as X ±SE.Correlations with metabolites of benign and malignant tumors and metabolites of normal muscles were done using Pearson's correlation coefficient. Results: 1.The means Cho,Cr,NAA and LL of benign tumors are separately (264.6±205.8)×10-6ppm ,(377.3±402.4)×10-6ppm ,(311.8±299.0) ×10-6ppm,and (2781.9±3029.0)×10-6ppm.The means Cho,Cr,NAA and LL of malignant tumors are separately(620.7±562.0)×10-6ppm ,(315.9±541.2)×10-6ppm ,(1858±4277.8)×10-6ppm and (1173.9±1694.1) ×10-6ppm. The means Cho,Cr and NAA of malignant tumors are higher than that of the benign tumors,but there are not significant differences between benign and malignant tumors of Cho,Cr and NAA(P>0.05). The mean LL of malignant tumors is lower than that of the benign tumors,but there is not significant difference between benign and malignant tumors of LL(P>0.05). 2.①The mean Cho/Cr ration of the malignant tumors(3.72±2.76) is higher than that of the benign tumors(0.88±0.53),but there is significant difference benign and malignant tumors of Cho/Cr ration(P〈0.01).②The mean Cho/NAA ration of the malignant tumors(2.08±2.43)is higher than that of the benign tumors(1.33±1.26), but there is not significant difference benign and malignant tumors of Cho/NAA ration(P>0.05).③The mean Cr/NAA ration of the malignant tumors(0.63±0.43)is lower than that of the benign tumors(1.52±1.36), but there is not significant difference benign and malignant tumors of Cr/NAA ration(P>0.05). 3.①There is significant difference between Cho and Cr in nomal groups (r=0.975,P<0.001),and therer is correlation between Cho and Cr in nomal groups.②There is not significant difference among metabolites of normal muscles in nomal groups. 4.①There are significant differences between Cho,Cr and Cr,NAA in benign groups (r=0.884,P<0.001;r=0.822,P<0.001),and therer are correlations between Cho, Cr and Cr,NAA.in benign groups.②There are not significant differences among metabolites of benign tumors in benign groups. 5.①There is significant difference between Cr and NAA in malignant groups (r=0.952,P<0.001),and therer is correlation between Cr and NAA.inmalignant groups.②There is not significant difference among metabolites of malignant tumors in malignant groups. Conclusion: 1.There is significant difference between benign and malignant tumors of Cho/Cr ration,and Cho/Cr ration is helpful to distinguish malignant tumors from benign tumors . Malignant tumors of Cho/Cr ration is elevated,and benign tumors of Cho/Cr rarion is fallen. 2.The means Cho,Cr and NAA of the malignant tumors are higher than that of the benign tumors, and the mean LL of the malignant tumors is lower than that of the benign tumors. There are significant differences between benign and malignant tumors of Cho,Cr,NAA and LL. So Cho,Cr,NAA and LL are not helpful to distinguish malignant tumors from benign tumors.
