Background: Acne vulgaris is a chronic and inflammatory skin disease involving in hair follicle and sebaceous gland that commonly occurs in adolescence and in young adulthood. It is commonly seen at department of dermatology, The major pathogenic factors involved are hyperkeratinization, obstruction of sebaceous follicles resulting from abnormal keratinization of the infundibular epithelium, stimulation of sebaceous gland secretion by androgens, and microbial colonization of pilosebaceous units by Propionibacterium acnes, which promotes perifollicular inflammation. The lesions are often seen on the face, back and chest because the sebaceous glands in these regions are particularly active. It is characteristic of papules, pustule, node, cyst, and. so on, with occurring repeatedly. Acne of ten appears on the surface of the facial skin, which has an an effect on the beauty. With the improvement of the living conditions, people attach more and more importance to the treatment of the acne.Western medicine regard acne as a multifactorial disease of the pilosebaceous follicle. Follicular hyperkeratinization is a key element in the pathogenesis of acne. The androgen stimulates the hyperplasia of sebaceous and the production of sebum, which is closely related to the occurrence, development and persistence of acne. In western meicine, the main drugs for treating acne include the drugs of anti-androgen, the drugs that can restrict the secretion of sebaceous gland and anti-keratinization, antibiotics, vitamine and so on. In spite of numerous treatments available, they have some side-effect, no ance therapies has been found that is security, valid and idealfor wowen up to now.In traditional Chinese medicine, acne vulgaris is generally called as "lung-wind acne" , It was recorded very early in Chinese literature. After systematically arranging historical records related to the pathogeny and mechanism of acne vulgaris, and carrying out an overview on literature in recent years, great advancement has been made in the understanding the pathogeny and mechanism of acne vulgaris in Traditional Chinese Medici ne(TCM). The doctor of TCM form some new point of view that "Xue Yu" , "Shi Re" , "Tan Jie" , "Shen Yin Bu Zu" , "Gan Yu" and so on. For women, acne often outbreaks associated with menstruation, which isrelated to "Gan Shi Shu Xie" and "Chong Ren Bu Tiao" . My tutor, Professor Fan Ruiqiang, in his long-term clinical practice, finds that the occurrence of acne vulgaris is closely associated with "Gan Jing Yu Re" besides "Shen Yin Bu Zu" and "Yin Xu Xue Re" . Based on the theory of above, Zi Yin Qing Gan Xiao Guo Fang(Fructus Ligustri Lucidi, Herba Ecliptae, Radix Bupleuri, Radix Curcumae, Radix Salviae Miltiorrhizae, Herba Houttuyniae, etc.) is prescribed by him to treat acne of wowen, and satisfactory therapeutic effects have been obtained.Purpose: In order to find a preparation of TCM, which has definite clinical effect and be convenience for taking, we use Single TCM concentrated Granules, of which Zi Yin Qing Gan Xiao Guo Fang Granules is composed. We study the clinical therapeutic effects of Zi Yin Qing Gan Xiao Guo Fang in treating female acne. 2. To study the prevalence of acne of high school students in Guangzhou, a cross-sectional study is performed in high school in Guangzhou, which will provide data and information for clinical study in the future.Method: 1. In the clinical study, 86 female patients with acne vulgaris were randomly divided into Zi Yin Qing Gan Xiao Guo Fang Granules group (57 patients) and Minocycline group (29 patients). The levels of hormones (T, E2) were be measured before and after treatment, and the clinical therapeutic effects of Zi Yin Qing Gan Xiao Guo Fang in treating female acne are be evaluated. 2. During December 2004, a sampling survey was proformed in seven high school in Guangzhou. 1561 students were involved in the study. Prevalence rate of acne, the age of onset of acne, treatment of acne and the degree of acquaintance with acne etc are investigated.Results: 1.
