Immuune function was a defensive ability of resisting infection, included immunologic defence immunologic homestasis and immunologic surveillance. Immunoglobullins and complements had a effection of resisting bacterium and virus in the body. As immunocompetent cell ,T lymphoxyte had a effection of immunologic surveillance. Immunoreaction of T lymphoxyte had important effection in resisting infection, NK cell was a lymphocyte, it had natural killeer ability and gave play to immunologic surveillance effection. It had recently been discovered that wounds of surgical operation could influence immune function. Now laparoscopic operation were rapid developed because it had a little wound and pain .Several abroad studies had reported about the Influence of laparoscopic cholecystectomy on immune system.Investigation and comparison changeable condition of laparoscopiccholecystectomy and open cholecystectomy on the immunoglobullins complements and T lymphoxyte. To study the Influence of laparoscopic cholecystectomy and open cholecystectomy on immune system.We selected 60 chronic cholecystitis patients. Laparoscopic cholecystectomy and open cholecystectomywere 30 patients. The patients had no diseases of cardiovascular system respiratory system alimentary system and endocrine system. The patients did not use drugs that affected immune function recently. 30 patients (13 men, 17 women, age range 31-72 years, median 45.6±9.8), who were put into effect laparoscopic cholecystectomy. 30 patients (12 men, 18 women, age range 26~-69 years, median 47.9 ± 8.6), who were put into effect open cholecystectom. There was no significantly difference of sex and years between two groups (p>0.05). Blood samples were obtained at study entry with preoperation and at day 1 5 postoperation. Samples were collected in plastic tubes. Serum was prepared to examine immunoglobullins(IgG IgA IgE IgM) complement(C3 C4) and T lymphocyte(CD,CD4,CD8,CD(16+56),CD19).The examination of immunoglobullins and complements selected reagent and instrument from the American DER Lin company,appling immunoradiometric assay. The examination of lymphocyte selected reagent and instrument from the American Kurete company and selected reagent from German company (IgGl-FITC/IgG!-PE, CD3-EITC/ CD4-PE,CD3-FITC/CD8-PE, CD3-FITC/CD(16+56)-PE, IgGl-PE, IgG2a-FITC. CD10-PE, CD3-PE,CD8-PE).Postoperative levels of serum immunoglobulins and complements of laparoscopic cholecystectomy and open cholecystectomy were lower than preoperative levels. The difference was not statistically significant (p>0.05). But the level of serum C3 was significantly decreased after open cholecystectomy, from 1.08 ± 0.15g/L to 0.96 ± 0.13g/L and 1.01 ± 0.12g/L, the difference was statistically significant(p<0.05 or p<0.01). The level of serum IgG was significantly decreased after open cholecystectomy, from 12.26±2.03g/L to 10.38±2.11g/L and 10.82+2.16g/L, the difference was statistically significant(p<0.05 or p<0.01). But the levels of serum C3 and IgG after open cholecystectom more significantly decreased than laparoscopic cholecystectomy, the difference was statistically significant (p<0.05 or p<0.01). Postoperative levels of serum CD8, CD( 16+56) and CD 19 of open cholecystectomy were little decreased than preoperation, the difference was not statistically significant(p>0.05). The level of serum CD3 was significantly decreased after open cholecystectomy, from 58.28 + 2.52% to 56.43 ± 3.98% and 52.05 ± 3.78%, the difference was statistically significant(p<0.05 or p<0.01). The level of serum CD4 was significantly decreased after open cholecystectomy, from 38.11+2.76% to 37.02 + 3.54% and 32.34 + 4.12%, the difference was statisticallysignificant(p<0.05 or p<0.01). The level of CD4/CD8 was significantly decreased after open cholecystectomy, from 1.59±0.36 to 1.46±0.32 and 1.41±0.28, the difference was statistically signiflcant(p<0.05).
