IntroductionMedulloblastoma is the most malignant tumor of nerve epidermic tumor in central nerve system, 3. 7% of the intracranial nerve epidermic tumor. As the tumor glows quickly, and there is the incline along the cranial spine fluid to bring the transmitted plantation, causing the treatment of this disease more difficult. So it is quite important to seek new prevention zealously and further improve the total treatment level of medulloblastoma in our country.Occurrence is caused by abnormal proliferation of cells, manifesting with biological behavior of autonomic growth out of control. Under the normal condition , the proliferation and apoptosis of cells maintain the homeostasis of the tissues , but once the cells occur abnormal proliferation or inhibition of apoptosis, all can cause malignant transform of cells without exception. Thereinto, the escaping mechanism of cellular apoptosis in the tumorous cells is the hotspot problem in resent study. Just as other malignant tumors, the occurrence and the development process of the medulloblastoma also exist in abnormal apoptosis mechanism. Survivin is the new member of inhibitor of apoptosis protein family (IAP). Our study substantiates that Survivin doesn't express in most end-point differentiation tissues, prompting that it may be through the inhibition of the apoptosis to the tumorous cells. To improve the occurrence and the development of tumor, we adopt the histochemistry method to detect the expression of the Survivin protein in the tissues of 65 cases medulloblastoma, to discuss the relative of Survivin in the medulloblastoma between adults and children, and the expres-sive relative of Survivin protein between the raedulloblastoma and the benign meningoma. Progressing the analysis of statistics, exploring the relative of Survivin and prognosis, and to supply the evidence for the choice of clinical therapy methods, and the judgement of prognosis.Method of testChoose the tissue of 64 cases medulloblastoma, which are cut off through surgery in the Neurosurgery of the Second College of China Medical University from January of 1993 to February of 2003. Thereinto, there are 11 cases of a-dults and 53 cases of children. Drawing out 10 cases meningoma of benign circulation in random. Routinely to prepare olefm, slice-up and the stain of HE, making the examination of pathologic histochemistry. The method of the stain a-dopting immune grouping method through the hypersensitivity reagent-box of S-P, and the slice-up is disposed through microwave. Using 0. 01nmol/L PBS to take place the first antibody as the blank contract, benign meningoma tissues as normal contract. The Statistics to the positive rate of expression and the level of expression about Survivin protein, adopt the semi-quantificational record-mark method and figure-analysis-ration method separately. The data of the test uses the software of SPSS10. 0 for windows to dispose in statistics.Result of testThe positive stain of Survivin protein all orientate in the cytoplast of medulloblastoma cells. The positive rate of Survivin protein expression in the tissues of 64 cases medulloblastoma, is 56.25% (36/(34) , while the positive rate of Survivin protein expression in the tissues of benign meningoma is zero, and there are obvious differences between them ( P <0. 001). The positive rate of Survivin protein expression in the tissues of adult medulloblastoma is 45.45% , while the positive rate of Survivin protein expression in the tissues of children medulloblastoma is 58. 49% , and there are no significant differences in the tissues of medulloblastoma between adults and children. Compared to the differences inthe survival curve between the positive expressive rate and negative expression rate in the Survivin protein. The average value of the absorption value in the tissue of 64 cases medulloblastoma expressed by the Survivin protein, is 0.0437 0. 033.5, while the tissue absorption degree of benign meningoma is 0. 04 0. 002, and there are significant differences between them ( P <0. 001). The expression levels of the Survivin protein in the tissues of adults and children medulloblastoma are no differences.DiscussionMedulloblastoma is the most malignant tumor of nerve epidermic tumor in central nerve system. At present, most statistics that the survival rate of 5years is beyond 30% to reduce the mortality of the patients with the medulloblastoma, improve the curative effect of the medulloblastoma, and develop the basic study of medulloblastoma actively, such as the correlative oncogenes, to supply the evidence and reference of the test for the direction of the clinical work.The regulative imbalance of the factors to improve the apoptosis and inhibit the apoptosis of cells, is the hotspot problem of the study to the occurrence and development of tumor recently. The inhibition of cell apoptosis is relative to the occurrence and development of malignant tumor, and not sensitivity to the chemotherapy and the radiotherapy. The further research to the inhibitive gene of the cell apoptosis has become the hotspot of molecular oncology. Survivin is the new member of cloned IAP family recently. The study exhibits primarily that Survivin is a kind of apoptosis inhibitors of the tumorous speciality and a new kind of the tumor markers. Survivin is further more ideal target-gene of the treatment to the tumor gene. The report of the Survivin protein expression in the tissues of medulloblastoma is rare. Chakravatri using the protein-imprint analysis method, detect the Survivin protein expression in the tissues of 92 cases frost colloid-tumor quantificationally, and find that the total survival stage of the patients withthe Survivin protein expression is obvious shorten ( P <0.001). The patients of GBM have the high positive rate and expression level of Survivin (P <0.001) , the total survival stage of Survivin positive patients is obvious short to the Survivin negative patients in the patients of GBM. Furthermore, the increase degree of Survivin protein expression is obvious relative to the survival time in all the patients of the glioma, the expression of the Survivin in the cranial glioma has the definite value of prognosis. Sasaki using the Survivin antibody of human, defects the expression of the Survivin protein in the tumors of 112 cases primary central nerve system, and find that most diffuse astrocytomas [ WHO II (2/4) III (3/3 ) IV (9/10) ] exist the immune response of Survivin protein. The intensity and degree of the Survivin protein expression is relative to the grade of tumors, and the expression is the strongest in the glioblastoma, while it appears focus positive expression in the medulloblastoma.We use the method of immunohistochemistry research the expression of Survivin protein in the tissues of 64 cases medulloblastoma, and analyze the relation between the Survivin protein expression and the clinical pathologic factors of medulloblastoma. We use the semi-quantificational method to the statistics of the positive rate to the Survivin protein expression. The result shows that the positive rate of the Survivin protein expression is 56. 25% , and the Survivin protein expression in the tissues of benign meningoma is negative. There are significant differences between them ( P <0. 001). It is shown that the increase of the Survivin protein positive expression is significant relative to the descending of the medulloblastoma survival rate, tliough the comparison of the survival curve differences between the negative expression and the positive expression of the Survivin protein. Use the method of the figure analysis to analyze the result of the immunohistochemistry quanlificationalry, to make the statistics of the Survivin protein expression level with the relation to the pathologic factors in clinics. The result shows that there are significant differences with the absorption value between the Survivin protein in the tissues of the medulloblastoma and in the tissues of the benign meningoma ( P < 0. 001). The expression level of the Survivin protein in the tissues of adult and children medulloblastoma, indicates that the expression degree of the Survivin protein in the patients of medulloblas-toma is significant relation to the ages of the patients.We use the semi-quantifieational method to judge the result of the immuno-histochemistry qualitatively, and use the figure analysis method to judge the result of the immunohistochemistry quantificationally. As a result, the qualitative analysis shows that the positive rate of the Survivin protein expression in the tissues of the medulloblastoma and the benign meningoma exists significant differences , while the quantificational analysis shows that the expression level of the Survivin protein in the tissues of adult and children medulloblastoma has no significant differences. The figure analysis can exclude the interruption of the background , and impersonally judge the absorbency of the stain domain to the immunohistochemistry, measuring the expression intensity of the guideline statistics-protein and avoiding the error caused by the subjective factors of the slice-readers. The result of our study has shown that the conclusion from the method of the figure analysis is more sensitive, and furthermore reflects the relation between the detected markers and the pathologic factors in clinics. Further testify the superiority of the figure analytic method.ConclusionThe positive rate of the Survivin protein expression between the tissues of the medulloblastoma and the tissues of the benign meningoma, exist significant differences. The positive rate in the group of the medulloblastoma is significant higher than the group of the benign meningoma, which indicate that the Survivin can improve the malignant transformation of the medulloblastoma, though the inhibition of cell apoptosis.There are no significant differences between adults and children in the expression level of the Survivin protein, indicating that the expressive degree of the Survivin protein in the patients of the medulloblastoma is no significant correlation with the age of the patients.The increase of the Survivin protein positive expression rate is significant correlation with the descending of the medulloblastoma survival rate, indicating that the expression of Survivin protein is significant correlation with the progno-...
