OBJECTIVE Aim of this study was to assess the diagnosis and therapy of myocarditis in patients presenting with acute myocardial infarction and normal coronary angiograms. MATERIAL Thirty-eight patients with clinical signs and symptoms consistent with acute myocardial infarction after angiographic identification of normal coronary, male in 13, female in 25. The clinical course and characteristics in all patients were analyzed. RESULTS reports have shown that myocarditis can mimic acute myocardial infarction with chest pain, electrocardiographic abnormalities, serum creatine kinase elevation and hemodynamic instability. The clinical condition was very serious in each patients, some was complicated with other organs injury. Verthicular enlargement occurred and dilated cardiomyopathy followed later in a few patients. Serum enzyme increased for a long time without enzyme peak, which presented as a character of the myocarditis. Coronary angiography isalways helpful in differential diagnosis between myocarditis and myocardial infarction, received immunosuppressive therapy is important for these patients. CONCLUSIONS Clinical diagnosis of mainly depends on patient's clinical sign and objective evidence of myocardial injury. Early diagnosis and successful treatment can be done .
