Severe acute pancreatitis (SAP), which is one common causes of acute abdominal pain , is complicated in processing, and often resulted in multiple local or systemic complications. So the patients with SAP have a high hospital mortality. During the past 30 years, we have known some about the etiology and pathogenesis of SAP, but they are still incertitude. It inspirited many people to face to the challenge, they put their intention on all domains of SAP, including the diagnosis, treatment and nursing care. Their purpuse is the earlier diagnoses, exactly judgements, validly intervention, alleviating pains, reducing the motality and making more patients rehabilitation. With an increasing number of patients surviving SAP, more attention has been directed towards health-relatedquality of life (HRQOL). Caring about SAP patients after discharge from the hospital, detecting their HRQOL, and discussing the health-related factors, all these combine the new kind of annotation of nursing conceptions. It is significative for this study under the new type of medical mode, which involved of physical, psychical and social aspects.ObjectivesTo evaluate the feasibility of the chinses version of 36-items short form health survey (SF-36) in assessment of the long-term HRQOL in patients after SAP surgery. To assess the long-term HRQOL of patients after SAP, who had received surgery more than 6 months before and discharged from hospital when they were charmed away. To investigate the impact of patients' demographic data, clinical characteristics in-hospital and the status of postdischarge on the different aspects of HRQOL .Methods62 patients after SAP treated by surgery, who were ever admitted in the general surgical ward or general intensive care unit between January 1998 and March 2004, at the Second Affiliated Hospital, College of Medicine, Zhejiang University, were identified retrospectively. Data regarding clinic characteristics, and the diagnosis, therapy and nursing processes in-hospital were obtained. The long-term HRQOL of patients after SAP surgery were assessed using the Chinese version of SF-36. And the postdischarge outcome of surgery after 6 months were asked by the appended 11 questions. The results of the survey were compared with an sex-, age-matched population. The differences between the two groups were analyzed by total scores and scores in 8 dimensions of SF-36.ResultsSF-36 showed good internal consistency (every Cronbach a of 8 dimensions was almostly more than 0.70) and repeatability. Convergent validity test and discriminating validity test were all succeeded (the rates were 100 per cent). SAP patients' long-term HRQOL total scores were general good, and similar to those of the sex-, age-matched population (P>0.01). But in the comparison of 8 dimensions of SF-36, the scores were significantly lower than those of general population in bodily pain, social function, role emotional and mental health (P<0.01). In multivariate backward regression analysis, there were 18 factors that had distinguishing relations with the 8 dimensions. Among the 18 factors , recall the experience in hospital, bodily uncomfortableness afterdischarge, follow-up time, degree of education, know about the cause of suffering, recurrent and outcome of treatment were the commom factors. And the three, recall the experience in hospital, bodily uncomfortableness after discharge and the follow-up time, were the most effectively ones. They had tight relation with some dimensions of mental health components, such as vitality, role emotional and mental health.Conclusions1. The Chinese version of SF-36 still showed good reliability and validity in assessment of SAP patients' long-term HRQOL , it can represent the status of life on physical health and mental health in SAP patients.2. SAP patients' long-term HRQOL total scores were general good compared with the sex-, age-matched population. But in bodily pain, social function, role emotional and mental health, the scores were significantly lower than those of general population.3. It was found...
