Objective:To investigate the preventive and treating effects of different therapeutic methods and its typical recipes on fatty liver ; provide theoretical basis for clinical treatment; develop the theory of pathogenic factor and pathogenesis, and therapeutic methods and principles of the fatty liver.Methods:The rat model of fatty liver was established by feeding high fat and sucrose diets combinated with white wine. Meanwhile soothing liver, invigorating spleen, tonifying kidney, dispelling dampness, promoting blood circulation, and complex recipes were given to treat different groups respectively. 10 weeks later, the pathological changes of liver tissue were observed under light and scanning electron microscope, and the contents of serum TC, TG, HDL-C, LDL-C, ALT, and AST were detected also.Results:1. Compared with normal group, the contents of serum TC, TG, ALT, and AST in model group were significantly increased(P<0. 01 or P<0. 05). After the intervention of medicine, the contents of serum TC, LDL-C, ALT, and AST in each treatment group were lower than those in model group, and there was a significant difference between soothing liver group and model group(P<0. 05).2. In model group, light microscope observation revealed fatty degeneration from medium to severe degrees in hepatocytes, swelling of hepatocytes, and different sizes of lipid droplets inside the cytoplasm. Infiltration of inflammatory cells and much focal proliferation of fibers were also shown in the portal area of model rats. Compared with the model group, the degrees of fatty degeneration in treatment groups were significant ameliorated, especially in invigorating spleen group. But the degree of fatty degeneration was seldom ameliorated in tonifying kidney group. Swelling ofhepatocytes, infiltration of inflammatory cells and proliferation of fibers in treatment groups were also ameliorated in different degrees.3. In model group, scanning electron microscope observation revealed a great deal of lipid droplets, swelling of mitochondria and endoplasmic reticulums in hepatocytes. The structure of hepatocytes in soothing liver group was nearly normal, there was little lipid droplets in it, and the structure of nucleus, mitochondria and endoplasmic reticulums became normal as well. The ultrastructure changes in other treatment groups were between those in soothing liver group and normal group.Conclusions:1. Improper diet, mal-adjustment of work and rest were the main pathogenic factors of the fatty liver. Stagnation of liver-qi, dyfunction of the spleen in transportation, and insufficiency of kidney were the main pathogenesis of the fatty liver, and functional disorders of liver and spleen were the key pathogenesis.2. Therapies of soothing liver, invigorating spleen, tonifying kidney, dispelling dampness, promoting blood circulation and complex were the main therapeutic methods of the fatty liver, and the therapies of soothing liver and invigorating spleen played important roles in preventing and treating the fatty liver.3. The results indicated that the above therapeutic methods and its typical recipes might have preventive and treating effects on fatty liver, such as adjusting lipid metabolism, ameliorating the hepatocytes fatty degeneration, protecting hepatocytes andorganellaes, ameliorating inflammation, and increasing liver function.
