Objective: By studying the effect of short-term preoperative chemotherapy on postoperative gastric emptying of cardiac cancer patients underwent surgical of proximal subtotal gastrectomy.we wanted to explore what role preoperative chemotherapy play in postoperative patients and to improve the life quality of postoperative patients. Gastric cancer has a poor prognosis. It is often diagnosed at an advanced stage, extensive resection and systematic lymphadenectomy the prognosis of patients with locally advanced gastric carcinoma remains poor[1]. There were data that showed neoadjuvant therapy in patients with locally advanced gastric carcinoma was feasible and appeared to increase the rate of complete tumour removal, decrease gastric carcinoma recurrence rates and increase survival[1-5]. But anticancer agents were toxic to body, and the long-term preoperative chemotherapy for cardiac cancer patients could induce side-effect of nausea,vomiting,anorexia,fatigue,stomatitis symptomatically and leukopenia thrombocytopenia,immunodepressio`n,infection,even MSOF[6] etc,those disadvantageoue factors maybe affect the surgery and the recovery of patients from operation. Whereas the short-term mild preoperative chemotherapy may decrease those[7]. It had deen estimated that the short-term preoperative chemotherapy with 5-Fu(Compound of Polyphase Liposome of 5-Fluorouracil ) might increase the postoperative survival rate after operation and be useful in patients with resectable gastric cancer[7-9],also enhance apoptosis in cardiac cancer after continuous intravenous infusion of 5-FU for 7 days[10]. The recovery of cardiac cancer patients were connected with gastric emptying postoperative. The gastric emptying pattern is determined by multiple factors. Any factors altered may affect gastric emptying. We wanted to analysis the effect with Compound Injection of Polyphase Liposome of 5-Fluorouracil to explored the relationship between short-term preoperative chemotherapy and the function disorders of gastric emptying postoperative of the patients who underwent surgical of proximal subtotal gastrectomy. The aim of the present study was to explore the possibility that the disorder of gastric emptying postoperative affected by short-term preoperative chemotherapy. We used radionuclide gastric emptying to evaluate the effect of short-term preoperative chemotherapy on postoperative gastric emptying of cardiac cancer patients underwent surgical of proximal subtotal gastrectomy.Methods: There were 37 patients with cardiac cancer were consecutively enrolled in this protocol (26 males, 11 females) with a mean age of 56.96±8.42 years (range 42~69).7 healthy volunteers who underwent laparotomy for esophagogastric reasons with a mean age of 52.22 ±8.01(range 39~63)served as control. The samples' pathological types: adenocarcinoma 31 cases, poorly differential adenocarcinoma 4 cases, adenoacanthoma 2 cases.No patients had received any chemotherapy or radiotherapy prior to the surgery. 37 previously untreated patients with gastric cancer were random divided into 2 groups. 21 patients(group1) received continuous intravenous compound injection of polyphase liposome of 5-fluorouracil(FPLC) at 160 mg/body weight/day for 7 days[7,10-11] preoperatively whereas the other 16(group2) did not receive any preoperative chemotherapy and the control(group3) received nothing. 21 patients were restaged 7-12 days after preoperative chemotherapy and then underwent surgical of proximal subtotal gastrectomy(no pyloroplasty or disconnection of pyloric sphincter by finger pressing) for cardiac cancer. 16 patients who did not receive any preoperative chemotherapy undergoing similar resections by the same surgeons. Before surgery all patients had radionuclide gastric emptying(group1 had radionuclide gastric emptying before chemotherapy). Approximately 30-40 days after curative surgery, all patients were scheduled to receive radionuclide gastric emptying.
