| Objective To investigate the distribution of hepatitis B virus (HBV) genotypes in Shan Xi province, to elucidate hot-spot mutations of HBV responsible for patient's state of illness infected with HBV.Methods HBV genotypes were measured by nest-PCR and restriction fragment length polymorphism (RFLP). We analyzed 147 infected with HBV. HBV genotype, levels of HBVDNA, mutations in the precore region at nucleotide 1862 and 1896, and mutations in the basic core promoter (BCP) region at nucleotide 1762/1764 were explored by single nucleotide polymorphism (SNP).Results Genotype B, C and D were detected in Shan Xi province. Genotype C and B were the majority genotypes of HBV with 131 out of 147 (89.1%) genotype C, 12(18.1%)genotype B and 1(0.7%)genotype D. Genotypes have no effect to degree of patients conditions, but can influence the frequency of hot-spot mutation. The higher level of HBV DNA more effected patient's illness than the lower level of HBV DNA. The mutation of each hot-spot was responsible for progressive liver injury in hepatitis B:OR of mutation of T1862 was e1.054=2.869, OR of mutation of A1896 was e0.915=2.497, OR of mutation of double mutations of BCP was e0.673=1.960, Patients who acquire double mutations of BCP and that of A1896 synchronously might render the progression, OR of it was e1.069=2.912.Conclusions Genotype C and B of HBV were the majority genotypes in Shan Xi province. Genotypes influenced the frequency of hot-spot mutation. Double mutations of BCP resulted in high level of HBV DNA and more seriously liver injury. The role of mutation T1862 to progress of hepatitis B was uncertain. Acquisition of mutation in the preC region at nucleotide 1896 aggravated the illness. Patients who acquire double mutations of BCP and mutation of A1896 synchronously might render the progression. |
