Objective based on the dates obtained from spiral CT scanning, to study the feasibility of guiding surgical planning and complication prevention using computer software.Methods 92 patients who suffered from bone disease and OSAS were scanned by bidirection spiral CT and data were obtained.Majority of the disease included tumour, fracture, congenital deformity,cysts of the craniofacial bone and OSAS. Data were sent to three-dimensional reconstruction work station to get 3D image. Then data were transferred to Navigator workstation to creat views similar to those displayed by fiberoptic endoscopy. The mimic operation was performed using 3D-cut software and compared with virtual situations in clinical operation process.Results The 3D-images from 3D-CT reconstruction showed all facial bones and their connections, confirm the borderline and size of the diseased tissues and its relation to the adjacent tissues. Three-dimensional mimic operation clearly displayed the relationship between tumour and surrounding tissues, and reflectedthe actual operation situation.Virtual endoscopy clearly displayed anatomic structure of the paranasal sinuses, nasopharynx and upper respiratory tract, and demonstrated the change of sinus' wall destroyed by bone tumor or fracture as well as the volume change of upper respiratory tract in OSAS patients.Conclusion Digital images based on Spiral CT scanning can help surgeons get more detail information about diseases. The images of virtual endoscopy and 3D-image are very clear, and 3D-cut can show every layer's situation. The clinical application shows that these method are very useful in surgical plan designing and predicting the potential complications before the actual operation.
