The combined transplantation of liver and kidney is a therapeutic option in case of end-stage liver and kidney disease.This dissertation consists of three part :A establishment of rat model renal allgraft ; A modified animal model of orthotopic liver transplantation in the rat; Establishment of combined liver-kidney transplantation model in rats.However the basic experimental research on combined liver-kidney transplantation is less ,the reason is that the technique is difficult and operation success rate is low.A rat is the better experimental animal in tranplantation research.Refer to related reports, This modified model of liver-kidney transplantation is stable and simple and feasible.Part one. An establishment of rat model of renal allgraftSD rats were served as donors and recipients.Kidney transplantation were performed in 160 rats .Infusion was conducted and prepared at prime position ,grafted kidney was put at left side. Direct vascular anastromosis and operation of connecting ureter attached part of bladder with the bladder were conducted.This results were as follow: After three month' s experience,we overcame the difficulties including artery anastomosis hemorrhage,venous thrombosis,unsatisfactory perfusion and genral peritonitis resulting from bladder anastommosis leakage. Operation success rate reached 85% or so.Recipients can survive 30 day. This modified model of kidney transplantation is feasible regardless of experimental conditionswith higher success rate and can be studied as the events following kidney transplantation.Part two. A modified animal model of ortbotopic liver transplantation in the ratOn the basis of cuff technique, we anastomosed the portal vein and subhepatic vena cava with cuff and conneted the suprahepatic vena cava to the recipient' s disphragm with 9-0 running suture.The results were as followed: The average time of acquiring donor liver was 32 minutes, reqairing liver was 12 minutes,unhepatic phase was 18 minutes.62 liver transplantation operation were carried out ,in which the operation success rate in Iate30 cases was 90%. The model is stable and can be used in the experiment of liver transplantation.Part three. Establishment of combined liver-kidney transplantation model in rats.SD rats were served as donors and recipients.In the donor operation,the liver and left kidney were flushed with 4tHC sodium lactate Ringer's inject through portal vein and aorta,respectively. Anastomosis of the portal vein and the inferior vein cava (IVC)inferior to the right kidney between the graft and the recipient was performed by a double cuff method ,then the superior hepatic vena cava with suture. A patch of aorta attached to the donor kidney were anastomised to aorta between the donor and the recipiend. The urethra and bile duct were anastomised using a simple inside bracket.This results were as follow: 54 combined liver-kidney transplantation operation were carried out ,in which the operationsuccess rate in late 30 cases was 76.6%. The function of the grafted liver and kidney remined normal. It is feasible that the model can be used to study the protective mechanism of the transplanted liver over the transplanted kidney.
