Objective: Of all the bladder cancer patients,20 percent of themare invasive bladder cancer, compared with the superficial bladder cancer the mortality is higher. Invasive bladder cancer own complicated biological behavior, many factors can affect its prognosis. We review 119 cases that take radical cystectomy for invasive bladder cancer, and analyze the survival time and clinical data by Cox model,then get the contributing factors that can affect the prognosis. Based on that ,we selectively discuss the management of invasive bladder cancer.Method: We review the clinical data and survival time of 119 casesthat take radical cystectomy and urinary diversion for invasive bladder cancer. After follow-up 89 cases of them have full clinical data and satisfying the requirement of Cox model. In our research, there are 25 predictive factors might affect the prognosis of invasive bladder cancer, these factors are as followsrcase type, duration of hematuria, the shape of tumor, the size of tumor, the site of tumor, involve bladder neck, involve trigone,involve prostate urethra, low urinary tract symptoms,distant metastasis , the number of tumor, anemia, wight lost, Tis, the type of urinary diversion, postoperative pathology, stage ,grade,lymph node metastasis and so on. We convert the clinical data as to the requirement of Cox model, and take solovariate Cox regression analysis.Result: There are 10 factors own highly statistic significance as to the prognosis of invasive bladder cancer,they are Grade(or=6. 07),stage (or=2. 31), Tis(or=4. 39), lymph node metastasis(or=5. 49) .involve prostate urethra(or=3. 26), distant metastasis(or=2. 68), lymph node metastasis number(or=2. 77), lymph node metastasis region(or=l. 95), low urinary tract symptoms(or=2. 82), weight lost(or=3. 41). Two facts own statistic significance as to the prognosis, they are pathology(or=2. 26)and the size of tumor (or=l. 58). Case type ,gender, age, duration of hematuria, shape of tumor, the number of tumor, involve post wall, involve top wall, involve anterior wall, involve trigone, involve bladder ncek, the type of urinary diversion will not affect the prognosis.Conclusion: There are several factors that act as determining roleas to the prognosis of invasive bladdr cancer, they are grade, stage, Tis, lymph node metastasis, involve prostate urethra, distant metastasis, lymph node metastasis number, lymph node metastasis region, low urinary tract symptoms, weight lost, pathology, and the size of tumor. We should improve the clinical practice as to these factors.
