Ischemic vascular disorders of the spinal cord are rare and there are different symptoms and signs when they onset. At present, MRI plays a very important role to diagnose ischernic vascular disorders of the spinal cord. However, it's easy to misdiagnose them when the imaging changes of other diseases are combined. In this article, we report a patient of ischemic vascular disorders of the spinal cord who was misdiagnosed as lumbar spinal stenosis. According to her clinical findings and imaging changes, we discuss the differential diagnosis between ischemic vascular disorders of the spinal cord and lumbar spinal stenosis cervical spondylotic myelopathy cervical disc herniation acute myelitis motor neuron diseases. We draw a conclusion that to diagnose ischemic vascular disorders of the spinal cord can't only depend on the image of MRI, but also should depend on the history of the disease and physical examinations.
