Objective The aim of the study is to make quantitative detection on the different degrees of cervical stenosis(cervical spinal stenosis)of cervical spinal cord lacertus patient(white matter fiber bundle)by magnetic resonance diffusion tensor imaging(diffusion tensor imaging,DTI).According to the experimental data of fiber bundles,to assess changes fiber bundles of the cervical spinal cord and the relation between the different degree of stenosis and the degree of cervical stenosis,and to find other quantitative indicators which reflect the degree of the spinal canal tenosis of cervical spondylosis.That mightprovide the reference for clinical evaluation of cervical spinal cord oppression degree.Methods Randomly 20 cases of 35-67 years old(average age 51.5,male 11,female 9)healthy volunteers are selected as normal control group,who were in DTI sequence scanning;the control group G0 consists of C4/5 C5/6 C6/7 volunteer disc level spinal cord DTI data.60 cases of 38-71 years old(mean age 49.6 years,33 men and 27 women)patients of cervical spinal cord compression who were diagnosed by MRI,were divided into mild group G1(20 cases),moderate G2 group(20 cases)and severe G3 group(20 cases)based on the reduction of sagittal diameter of spinal canal.Three group patients were in the routine MRI scan and DTI scan,JOA scan.DTI score,Philips EBW Extended MR the original data by Workspace4.0 workstation Fiber Trak Define Postprocessing software Multiple ROIs tools to rebuild the cervical spinal cord white matter DTT image;the normal control group were measured and recorded the corresponding intervertebral disc plane,ADC,Lines,FA Length data;cases were measured and recorded the lesion of spinal canal sagittal diameter and adjacent normal vertebral canal sagittal diameter,FA,ADC,Lines,Length,JOA score.The SPSS19.0 statistical software was applied in the whole process,and the above data were statistically analyzed.P < 0.05 was statistically significant.Results Clear images were obtained in all of the above groups.G0 group C4/5,C5/6,C6/7,intervertebral disk plane,FA,ADC,Lines and Long single factor analysis of variance,FA(P>0.05),ADC(P>0.05),Line(P> 0.05),Length(P> 0.05).G0 group of male and female gender matched test FA(P>0.05);ADC(P>0.05)and Line(P > 0.05)and Length(P > 0.05),G0 group and G1 G2 G3 analysis of variance analysis and comparison of 22 FA(F=49.026,P< 0.05);ADC(F=34.443 P< 0.05);Line(F=1.880,P> 0.05),Length(F=0.677 P>0.05).G1 G2 G3 group DTI parameters and cervical stenosis degree of correlation analysis,FA(r =-0.579 P < 0.05);ADC(r =0.677 P< 0.05),Line(r =-0.202 P> 0.05),Length(r =-0.165 P> 0.05);G1 G2 G3 analysis of DTI parameters and JOA score correlation FA(r = 0.663,P< 0.05);ADC(r =-0.716 P< 0.05),Line(r= 0.203 P> 0.05),Length(r =0.161 P >0.05).Conclusion1.Normal adult cervical spinal cord C4/5 C5/6 C6/7 disc level FA and ADC values are different,and from top to bottom gradually change.There are no significant changes on FA and ADC values,neither line value nor long value is not statistically significance in the disc plane.2.FA value,ADC value,line value and length value are different in different degree of cervical stenosis.With the increasing of stenosis degree,ADC value increases gradually and FA value decreases gradually.The difference is statistically significant,which can provide the reference index for the cervical spinal stenosis.3.The FA value,ADC value,Line value and Length value are different in the cervical spinal stenosis patients with different spinal cord compression.FA and ADC are correlated with the JOA score,which can point out the compression degree of the cervical spinal cord.There is no correlation between the scores of Line and Length and the scores of JOA,which is independent of the degree of oppression.FA and ADC can be used as reference indexes for judging spinal cord compression.4.It shows the cervical spinal cord compression as FA value is 0.642±0.43 and the ADC value is 1.026±0.084.When the FA value is 0.537±0.57 and the ADC value was 1.275±0.071,it indicates that the degree of oppression is serious,the FA value gradually decreases,and the ADC value increases gradually,which indicates that the disease is aggravated,and conversely,it indicates that the disease is improving.
