At present, radioactive treatment is still the main method for the treatment of middlE-late phase pulmonary carcinoma. We collected 58 III phase pulmonary carcinoma cases which proceed radical radioactive treatment using 3 - DCR technology, and detect the microvascular density ( MVD) and E-cadherin ( E-CAD ) expression level of the prE-radioactive biopsy sample by using the method of immuno - histological chemistry, then analysis the relationship of these factors with the radioactive sensitivity of pulmonary carcinoma; Discuss their application value in the area of exactly predicting radioactive sensitivity of pulmonary carcinoma prior to radiation; and provide the theoretical basement to the personal protocol of the radioactive treatment of pulmonary carcinoma.PurposeSupposing we can correctly predict radioactive sensitivity of tumor before radiation and design personal therapeutic protocol, may be the key point for improving tumor's local control rate and radiative curative effect of III phase pulmonary squamous cell carcinoma. Many studies find that biological factors may have relations with tumor' s radioactive sensitivity. Therefore we observe the, correlation of MVD and E-cad expression level with the radioactive sensitivity of pulmonary squamous cell carcinoma, and the relation between MVD and E-cad of pulmonary squamous cell carcinoma, to expect to find a biological index for exactly predicting the radioactive sensitivity of pulmonary squamous cell carcinoma.Material and methodFrom 2002 - 8 to 2004 -1 in the CMU No. 1 hospital 58 middlE-late phase pulmonary squamous cell carcinoma patients with definite pathologic diagnosis complete full - term radical radiation therapy. We use immuno - histological chemical method to detect MVD and E-cad expression level in the bronchoscope biopsy sample before radiation and observe the dwindling of the tumor after the patient receive radiation according to 1981 WHO standard objective grade for recent curative efficacy of substantial tumor; analysis the correlation of MVD and E-cad expression with radioactive sensitivity of pulmonary squamous cell carcinoma. As for statistic method, we adopt SPSS 10.0 statistic analysis software, and according to the character of data, perform t - test, chi - square test, Spearman grade related analysis and multiple linear regression analysis. All results have statistic significance when P <0.05.ResultMVD is tightly associated with lymph node ( LN) metastasis of pulmonary squamous cell carcinoma; MVD in the LN metastasis positive group is much higher than in the LN metastasis negative group(t = 2.073,P < 0.05). E-cad is tightly correlated with both differential degree and LN metastasis of pulmonary squamous cell carcinoma. Following the descending of the differential degree, the positive rate of E-cad decreased obviously ( x2 = 6.49, P < 0.05 ) ; in the LN metastasis positive group, E-cad positive rate is obviously lower than the LN metastasis negative group (x2 =4.23 ,P <0.05) , and is also abnormal expressed. E-cad expression is significantly negative associated with MVD in the tissue of pulmonary squamous cell carcinoma(x2 = 16.81,P<0.05). Radioactive sensitivity with high MVD is obviously higher than with low MVD (x2 =5.24,P <0. 05 ) ; radioactive sensitivity with low E-cad positive rate is obviously higher than high E-cad positive rate(x2 =5.98,P <0.05).DiscussionFor many years, people have already known that tumor radioactive sensitivity is associated with intra - tissue oxygen content, and oxygen content is tightly related with the number of the tumor - bed vessels. For the tumor tissue, vessel is nutritious, blood flow is good, and oxygen is much, so the tumor's radioactive sensitivity may be high which the therapeutic efficacy may be good. MVD is the most ideal index to reflect tumor vascular genesis and one of the important variables to know about intra - tumor oxygen content.
