| Objective To investigate the relation of serum hyaluronicacid(HA),Hepatocellular apoptosis, Child class and cirrhosis ,postoperation recovery of partial hepatectomy patients .The relationof these indexes and liver reserve function are also studied.MethodsFrom sep.2002 to oct.2003,54 cases underwent partialhepatectomy,include 10 cases hemangioma(control groups),32 caseshepatocellular carcinoma and 12 cases cholecystolithiasis. Accordingto ascites, jaundice and hepatic encephalopathy,The standard of therecovery of hepatic functions at postoperation were divided into good,midworst ,worst group,The blood of all patients were taken for thedetermination of serum hyaluronic acid and regular hepatic functionat preoperation ,midoperation and postoperation 1 days and 7days,respectively.The liver tissue were taken for examination ofcirrhosis and apoptosis. Cirrhosis and the standard of the recoveryof hepatic functions at postoperation were analysised with serumhyaluronic acid , hepatocellular apoptosis indexes(AI) and Childclass.Result Serum HA levels of the hepatopathy groups significantlyhigher than control group(F=28.288,p=0.000). AI of the hepatopathygroupsweresignificantlyhigherthancontrol group(t=7.087,p=0.000),AI werealso positively correlated with HA,There were 24 cases of cirrhosisin 54 cases patients,AI and HA of the cirrhosis groups weresignificantly higher than non-cirrhosis groups(t,=2.3871, 4p<0.05;t,=2.9661,p<0.05). Two groups were divided according to the95% confidence interval of HA in cirrhosis group ( HA<155,HA≥155), Cirrhosis and the standard of the recovery of hepatic functionsat postoperation were predicted, The accurate rate were 79.55% and70.45% in HA<155,HA≥155 group, respectively ,there were significantlyhigher than that of Child class group(x2=4.3279,p=0.0373x2=3.8426,p=0.04). Two groups were divided according to the 95%confidence interval of AI in cirrhosis group ( AI<0.0381,AI≥0.0381), The accurate rate were 79.55% and 72.73% in AI<0.0381,AI≥0.0381 group, respectively ,there were significantly higher thanthat of Child class group(x2=4.3279,p=0.0194; x2=4.793,p=0.0285). Twogroups were divided according to the 95% confidence interval of AI andHA in cirrhosis group (AI<0.0381,HA<155;AI≥0.0381, HA≥155) ,Theaccurate rate were 87.18% and 84.62%, respectively ,there weresignificantly higher than that of Child class group(x2=8.1428,p=0.0043; x2=11.819,p=0.0009). The accurate rate were59.09% and 50% in Child class groups,respectively,this chang was notin absolutedly good accordance with Child class. Conclusion HA and AIcould serve as the sensitive indicator for predicting the chang ofliver function and liver reserve function. |
