Objective To investigate the prealbumin(PA) test for assessment of liver function damage and liver reserve ability. Method The PA was tested dynamically in the course of experimental inducement of hepatocirrhosis in rats, the correlation between PA and routine liver function test, degree of hepatocirrhosis and MEGX were analyzed. The peri-operative PA was examined in 120 cases with primary liver cancer (PLA) underwent left hepatectomy and the correlation between PA and routine liver function test, the degree of hepatocirrhosis and hepatic failure were analyzed retrospectively. The peri-therapeutical change of PA and the post-therapeutical effects on PA change of various factors, such as times of treatment, volume of lipidol, with or without portal cancer thrombus, were investigated in patients accepted TACE. Results The serum value of PA can sensitively and exactly reflect the reserve ability and damage degree of liver function in experimental hepatocirrhosis in rats. There was a positive correlationship between the changes of PA and MEGX in the course of experimental inducement of hepatocirrhosis(r=0.67,p<0.001). The reduced degree of PA was corresponded with the Child grade of liver function and the cirrhotic degree of liver in patients with PLA. It was precarious to the patients whose pre-operative serum value of PA was less than 10mg/dl, and the incidence rate of liver function failure was high if the post-operative PA was less than 8mg/dl. In patients accepted TACE, PA could exactly reflect the liver function damage cause by various factors, such as times of treatment, volume of lipidol, with or without portal cancer thrombus. Conclusion The serum value of PA can sensitively and exactly reflects the liver reserve ability and liver function damage. It has clinical value in choice of operation, prediction of hepatic failure after various treatmentS and prognostic judgment of liver failure by testing PA.
