Objective: To evaluate the clinical feature and 1H-MRS alteration of dementia with Lewy bodies for the purpose of early diagnosis, differential diagnosis and effective therapy.Method: We analyzed the clinical feature, sign, neuropsychological examination and auxiliary examination of 6 cases of dementia with Lewy bodies. 1H -MRS examination was performed in 5 of these cases. Concentrations of NAA, Cho and Cr were evaluated. Results: Dementia with Lewy bodies is more common in the male than the female (5/1). The average age of onset is 71.5. The clinical manifestation includes fluctuating cognitive impairment (4/6), recurrent visual hallucinations (5/6), spontaneous motor features of Parkmsonism (5/6) and REM (rapid eye movement) sleep behaviordisorder (2/6), as well as repeated falls, delusion and acoasma. The 1H -MRS alteration of these 5 cases includes significant reduction of NAA/Cr ratio with no significant change of Cho/Cr ratio. Conclusion: Fluctuating cognitive impairment patient with recurrent visual hallucinations and spontaneous motor features of Parkinsonism shall be high suspected to be dementia with Lewy bodies. 1H -MRS alteration indicates the loss of neuron, which may be helpful in the early diagnosis, disease course monitoring and therapeutic effect evaluation of this disease.
