| AIM : To evaluate the clinical diagnostic value of heart rate variability (HRV) and Poincare plot for coronary heart disease (CHD). METHODS: There were 250 cases be selected , of them, 108 cases of angina pectoris , 60 cases of old myocardial infarction , 52 cases of CHD be verified by coronary angiography (CAG) and taking another 30 without CHD confirmed by CAG as controls. We analysised and compared following factors: sex, age, hypertension, glucose, uric acid, total cholesterol, triglyceridemia, segmental movement abnormality, time domain of 24h HRV and Poincare plot. RESULTS: (1) Compared with control group, shows the age, percentage of mankind, hypertension, high blood glucose, serum uric acid, triglyceridemia and segmental movement abnormality were much higher than controls, but SDNN, VLI, the proportion of comet-shaped Poincare plot were lower obviously. (2) While the aggravation changes of severity of coronary arteries, the SDNN, VLI and proportion of comet-shaped Poincare plot were lower obviously than clinical data. (3) The sensitivity and specificity of Poincare plot are 92.2% and 56.7%, of SDNN are 50% and 86.7% respectively. In addition, by Logistic regression analysis of SDNN, sex, Poincare plot, hypertension, hypertriglyceridemia, segmental movement abnormality its data were usefulness of diagnosis of CHD. CONCLUSION: The grade of functional lesions of autonomic nervous of cardiac system correlated well with coronary arteriolpathy, both influenced together too much. SDNN and Poincare plot can be areference way in clinical diagnosis of CHD and Poincare plot is more audio-visual and more sensitivity than SDNN. |
