ObjectsMycoplasma pneumoniae ( MP) is a pathogenic agent responsible for numerous outbreaks of acute respiratory infection in children. M. pneumoniae pneumonia (MPP) is known as one of the respiratory infectious diseases found mainly in children. The incidence of MPP in children has gone up year by year. Themechanism of MPP has not been clarified up to the present, but a large amount ofevidences have shown the possibility that the immune responses play a significant role in the development of MPP. T helper type cells can be subdivided into 2 subsets, T helper type 1 (TH1) and T helper type 2 (TH2) , based on their cytokine profiles. TH1 cells are characterized by the secretion of interferon (IFN)- and promote cell-mediated immunity. TH2 cells selectively produce IL-4 and are involved in the development of humoral immunity. It has been suggested that the unbalance of TH1/TH2 immune response may results different diseases to be occurrenced . Data on the cytokine profile in terms of TH1 or TH2 type in M. pneumoniae infection are scarce at home and abroad,and the research of IL-4/ IFN- variety in the sputum has not been reported. Sputum induction has the good relativity with various index signs in that bronchoalveolar lavage fluid ( BALF) examine. Induce sputum technique has been developed in recent years , it has safety, untility, noninvasion, and wonderfull repetition. In this study, we investigated the contribution of IL-4, IFN-'yin serum and in induced sputum in children with MPP.Methods1. Patient subjects: The study participants included 38 patients with Myco-plasma pneumonia, 20 controls between February and August of 2003 as part of an approved study protocol or for clinical indications. The Mycoplasma pneumonia group who had a clinical condition that necessitated hospitalization. The diagnosis of MP was based on clinical ( fever, cough, dyspnea, rales, etc) and radiologic (chest infiltrates) criteria, and on the antibody -IgM titer of at least 1:40 . There was no significant difference in patient group and normal control group in age and sex.2. Methods: (1) The collection of serum sample: Fasting vein blood (2 ml) was collected under sterile conditions from every child to analyze. The specimen is centrifurged immediately at 3000 r/min for 5 minutes . The obtained supernatant was loaded on reserved at 70掳C prior to IL-4 and IFN- assay. (2) The collection of induced sputum; The method of sputum induction is refer to Pin's. Induced sputum was collected respectively in acute episode period group (24cases)and in remission period group (20cases). The induction was carried out with an aerosol 3% of hypertoil saline by an ultrasonicnebulizer, total < 30min. After inhalating, the child is asked to cough and try to expectorate sputum into a cleaning plat. Selecting qualification sputum 1ml . The specimen is treated with sputum protectional liquor, produced the China Medical University , to break up the mucus and disperse the cells, and centrifuged at 2000r/min for 10 min,The supernatant was stored at -70, for future analysis. (3) The concentrations of these cytokines were determined in serum and sputum supernatants using an ELISA kit .3. Statistical Analysis; Data are expressed as mean standard deviation ( x s) . Comparison of mean were made with student 's t test. Use linear correlationanalysis in calculation the related coefficient. Judge the correlation degree in each measurement. For all other outcomes, a P value of 0. 05 or less was considered to indicate significance.Results1. Serum IL-4 levels were significantly higher in the acute episode period in MPP group compared with children in remission period and the controls ( P <0.01). Serum IL-4 levels in the remission period was not different from the control group( P >0. 05). Serum IFN- 7 concentration in the acute episode period softly higher than that in remission period and the controls, between which it did not differ significantly ( P >0.05).
