Objective To study the interrelationship among resistin, IL-6, carotid intima-media thickness(IMT) and insulin resistance in type 2 diabetic patients , and therapeutic effects of pioglitazone. Methods Resistin ,IL-6 levels were detected by enzyme-linked immunosorbent method and IMT was assessed by a high resolution doppler ultrasonography in 48 patients with type 2 diabetes and 44 healthy controls. Diabetic patients were allocated at random into the treatment and placebo groups.Compared with the changes of above indexes, pre and post treatment with pioglitazone or placebo 30mg per day for 3 months . Results (1)The levels of resistin , IL-6, IMT and insulin resistance index (IRI) were significantly higher in type 2 DM than in normal controls. (2)Compared with placebo group, the levels of resistin, DL-6, EMT and IRI were all significantly decreased on treatment group after treatment with pioglitazone for 3 months. (3)The level of resistin showed positively correlated with IMT. The change of resistin was closely positively correlated with the change of IL-6 pre and post treatment in diabetic patients. IL-6 was positively related with IMT in diabetic patients accompanied by cardiovascular disease. IRI was positively correlated with BMI, WHR, TC, LDL-c and IL-6. Interpretation Type 2 DM existed hyperresistinemia and hyperinterleukinemia-6 and IMT was significantly thicker than healthy controls. Resistin, IL-6, IMT were mutually influenced and closely interrelated to their parameters as clinical course of diabetic disease. The pioglitazone therapy can significantly decrease the levels ofresistin , EL-6, IMT, IRI; help to prevent and improve the setting as well as development of atherosclerosis and vascular complications.
