With the application of high-powered weapons in modem wars and the development of transportation, mining and construction, fracture of pelvic and acetabulum become more and more frequent than ever before. For the complex anatomy and often accompanied with compound injuries, such as injury of viscera in pelvis, the death rate is much higher. Traditionally, without serious compound injury, the fracture is treated conservatively. Thus it often makes the patients feel worse, especially those with acetabulum fracture. Many doctors believe that in order to improve patient's life quality and reduce death or invalid rate, replacement and fixation should be the best choice. For some reasons, clear image exam and accurate diagnosis will be more decisive. In this study, we designed a database for collection of case data and arranged for diagnosis and treatment of pelvis andacetabulum fracture.Part 1: The design of pelvis database and related studyObjects: 1 .To set up a series of database systems for collecting clinical data.. 2.To analyze clinical data by the database: evaluating the different diagnosis veracity of different image exam methods. Methods: Inpatients of pelvis and acetabulum fractures from April 30th 2001 to April 30th 2004 in department of orthopedics in Xijing hospital were chosen as orthopedics research objects. Patients who died without image exam or be in hospital less than 24 hours were excluded from our study. The diagnosis veracity of X-ray and Two- dimensional CT and Three-Dimensional CT (spiral volumetric CT and electron beam CT) were compared by operation. Results: The diagnosis veracity of Three-Dimensional reconstruction CT is much higher than that of X-ray and Two- dimensional CT.Part 2: The clinical use of Three-Dimensional reconstruction and rapid prototypingObjects: To observe the clinical guiding effects of combined operations of Three-Dimensional reconstruction and rapid prototyping. Methods: 1 .The cases of database were examined by spiral volumetric CT or electron beam CT. 2.The data were transformed and put into rapid prototyping machine. 3.The pelvis model was made as the same size as the real pelvis and simulating the injury. 4.Clinical treatment was guided according to the models. Results: 15 cases of pelvis and acetabulum fracture had been treated with the guidance of Three-Dimensionalreconstruction and rapid prototyping since October 2003 in the department of orthopedics, Xijing hospital. Conclusion: 1.The data of Three-Dimensional reconstruction can be transformed to the data of rapid prototyping machine in gear. 2.The pelvis model made by rapid prototyping machine can simulate the condition of real pelvis to guide clinical treatment.
