Objective: To determine the effect of apoptosis on hepatocellular carcinoma of rats after the treatment of transcatheter arterial chemotherapy (TAC) and portal vein chemotherapy(PVC) with octreotide (Oct); To compare the effect on rats implanted liver tumor after TAC+PVC with Oct or 5-fluorouracil (5-Fu);To prepare for clinical application of Oct.Methods: 60 male SD-rats were enrolled into this study. Half of Them were randomizied into three groups, each group contained 10 rats. Group A received treatment of TAC+PVC with Oct, group B received treatment of TAC+PVC with 5-Fu and group C received treatment of TAC+PVC with normal saline(N.S),respectively.7d after treatment, all rats were killed, every group's tumor volume and immunohistochemical examination of TUNEL﹑Bax﹑Bcl-2 were determined and compared each other. The others were randomizied into three groups and received treatment like before, To determin the survival time of every rat and make a survival analysis.Results:7d after treatment, the average suppressed tumor ratio is 62.12% in group A and is 59.44% in group B, respectively(P>0.05).Tumor volume of group A and group B show a significant reduction at 7d after treatment versus group C, respectively(P<0.05).TUNEL: results of group A and group B show a significant elevation versus group C(P<0.01);result of group A show a significant elevation versus group B(P<0.01). Bax: result of group A show a significant elevation versus group C(P<0.01),it is same as group A versus group B(P<0.01);while there is not significant difference between group B and group C(P>0.05).Bcl-2:results of group A and B show a significant reduction versus group C, respectively(P<0.01), it is same as group A versus group B(P<0.01).It also shows a significant difference between group A and B after immunohistochemical examination of TUNEL﹑Bax and Bcl-2 in normal liver tissue beside the hepatocellular carcinoma(P<0.01).Through electron microscope, apoptosis was observed obviously in tumor nucleus and mitochondrion of group A; Slight changes of apoptosis was observed in group B, while there were no significant changes of apoptosis observed in group C. After TAC+PVC, the longest survival time is 39d that come from group A, while the shortest survival time is 22d that come from group C. There is significant difference about survival time between group A and B(P<0.05);There is not significant difference about survival time between group B and C, and so does it between group A and B.Conclusions: TAC+PVC with Oct is able to inhibit the proliferation of liver tumor and promote apoptosis of hepatocellular carcinoma in rat; Oct's anti-tumor effect is better than 5-Fu, while damage to nomal liver tissue is weaker than 5-Fu. The survival time of group A is more longer than group C. It is not difference about survival time of rats treated by Oct or 5-Fu.
