Objective To investigate the influence factors of casual blood pressure (BP) variability and effect of arm circumference on blood pressure with three measurements. Methods A total of 2020 urban population was divided into 5 groups by age,≤35 years,36-years,46- years,56- years,>65 years,male:994,female:1026. BP was read by a senior experienced physician using same calibrated mercurial sphygmomanometer, all measurements were taken at right arm after patients sitting for 15 minutes, for each thirty seconds BP was sampling once, the sampling number was measured three times, at same time recorded systolic blood pressure (SBP), diastolic blood pressure (DBP), weight, age, sex, high, hip circumference, waist circumference, right arm circumference (AC). The SBP variability(ΔSBP) and DBP variability (ΔDBP) were the difference of maximal and minimal SBP or DBP respectively. Unpaired 2-tailed t test was performed to assess differences between BP parameter of male and female, linear correlation and multiple linear regression were used to evaluate relation of AC with BP measurement. Results 1. SBP rose with the increase of age after 46-group,the SBP of female was significantly higher than that of male after 46-group,DBP decreased with the increase of age. DBP of female was significantly lower than that of male before 65 years old. Pulse pressure rose with the increase of age. 2.Within 35-65 years old,ΔSBP of female was significantly higher than that of male. The ΔDBP showed little difference between female and male. 3. The percent of the maximal SBP of male and female in first reading were about 48.5% and 53.1% respectively. The percent of the maximal DBP of male and female in first reading were about 44.3% and 49.6% respectively. TheΔSBP of the group of the maximal SBP in first reading was significantly higher than others. The peaks of ΔSBP and ΔDBP were 52 mmHg and 20 mmHg respectively. 4. The SBP and DBP were significantly correlated with AC after 36-group. Compare with AC and WHR, BMI was most important impact factor on SBP, but on DBP AC was most important impact factor. Conclusion 1. Casual BP variability is very significant. The highest of ΔSBP and ΔDBP are 52mmHg and 20mmHg respectively. 2. BP variability of female is more significant than that of male, particularly in menopause, the aging is one of impact factors to ΔSBP. 3.ΔSBP is highest when the maximal SBP in first measurement. 4. AC is one of important impact factors on BP measurement, especially to DBP,so the BP must measure by compatible cuff belt. |
