| Objective: To study the expressions of Aquaporin(AQP1,2,5)in nasal polyp,sinusitis and perioperative period of FESS;and to study the influence of FESS on the expressions of AQP1,2,5,so as to clarify the significance of AQP1,2,5 inducing more nasal secretions and edema,in order to put forward some theoretical viewpoints which could be adopted in the postoperative recovery.Materials and Methods: According to the diagnosis and curing criteria of chronic sinusitis(1997,HaiKou), 20 cases of type I and 20 patients of type II were enrolled(all individuals underwent the endoscopic managements after FESS, and none of them had the medication of corticoid), at the same time, other groups also classified based on various stages during the perioperative period:preoperative group, 1-2-week group after FESS, 4-6-week group and 8-12-week group, each group was made up of 20 cases; control group was set as well which were the patients with septal deviation inspite of other diseases. All samples were taken from the mucosa guided by endoscope: preoperative samples were taken intraoperations, postoperative speciemen came from reserved mucosa in operative field; those of the control group were removed in the musoca of anterior part of normal inferior turbinate. Underwent the HE stains,immunohistochemical techniques and Western blot, morphological changes of nasal mucosa,expressions and distributions of AQP1,2,5 and alternations of relative quantities of perioperative AQP1 in the mucosa of different study groups were observed and measured.Results: 1.Mass morphological observations of rhinomusoca during the perioperative period:(1)P1 and S1:discharge could be seen in nasal cavity, accompanied with mucosa congestion and edema;(2) P2 and S2: nasal discharge decreased, congestion and edema improved compared to P1 and S1, while some tiny vesicles occurred; (3) P3 and S3: with less discharge, almost eliminated edema and primary epithelialization.2.Location express of various subtypes of AQP1,2,5:(1)AQP1 expressedmainly in cytoplasm of mucous gland cells,and slimly in epithelial lining of the normal nasal mucosa;(2)AQP1 expressed both in cytoplasma and intracavital membrane of the mucous gland cells,and also had a weak expression in epithelial lining of nasal polyps;(3)AQP1 expressed in cytoplasma of mucous gland cells and didn't in epithelial cells from the inflammatory sinal mucosa;(4)In the cytoplasma and cell membrane of the mucous gland cells from reserved mucosa after FESS,AQP1 had expressed,while they didn't in epithelial cells;(5)AQP2,5 had a negative expression in all trial groups.3,AQP1 countings in positive cells of all trial groups(which was demonstrated by :(1)Nasal polyp groups:difference between P0 and P1 didn't have significance(Pï¼0.305>0.05)ï¼›which hadn't between P3 and control group(P>0.05)ï¼›which among other groups had all signficance(P<0.01)。(2) Sinusitis groups: difference between S0 and S1 didn't have significance(P>0.05)ï¼›which hadn't between S3 and control group(P>0.05)ï¼›which among other groups had all signficance(P<0.01)。4,Protein blotting results: AQP1 appeared in the samples of every group, anti-AQP1 antibody had shown that: positive stains occurred at 28Kd and 35-45Kd respectively; and they matched the non-glycosylated protein and glycosylated protein correspondingly. A value of AQP1 in those groups were measured as . P values of comparisons among the P0,S0 and P1,S1; among P3,S3 and control group were greater than 0.05; on other occasions, P were less than 0.01.Conclusions: (1) AQP1 might be a vital factor for the formation and elimination of mucosal edema in operative fields after FESS;(2) Changes of AQP1 expression might be a participant in the alternations of nasal discharges after FESS; (3) Single AQP1 in nasal mucosa might be unable to maintain the common water transportation in mucous gland epithelium and epithelial lining under the influence of various pathogenic factors,which might be one of the reason why edematous polyps readily locate in nasal mucosa. |
