Objective: During the treatment of hepatic tumors, especially operation,acute liver function failure are a kind of severe complication.We establish the acute ischemic liver function failure of the porcine to study the character of acute liver function failure and make an evaluation of the effect of an in vitro perfusion supportive therapy of the biotic liver. As the requests for our experiment are convenient performance,easy to control and after the model is established,it must get into the acute liver failure stage very quickly, besides, the liver of the model itself has little effct on the in vitro liver,thus,we can evaluate the function of the in vitro liver through the status of the recipient, we choose operation to establish the model.Also,we can study the mechanisms of the onset and death of acute liver failure.Methods: Laboratory animals(n=8) are all common healthy porcine with a weight between 20kg and 30kg.Before operation ,atropine 0.05mg/kg and ketamine hydrochloride 1 Omg/kg,i.m,then we perform basal anesthesia and establish a venous circuit through the otic edge vein of the landrace.After the induction of anesthesia through ketamine hydrochloride and disoprofol i.v..tracheal cannula and inhalation of general anesthesia are performed and nitrous oxide, ethrane are inhaalated to maintein anesthesia. Establishing a central vein and begin to measure the central venous pressure through external or internal jugular vein. Put invasive surveillance device of arterial pressure on the commom caroid artery.After opening the abdominal cavity,we perform ligation of the portal vein and the hepatic artery,then perform the diffluence of the portal vein and the inferior vena,close the abdominal cavity layer by layer after bladder fistulation.At last, we can observe the general condition,survival duration, serum biochemistry,serum ammonia,blood coagulation after the lanclraces recover all the autonomous breathing and have the tracheal tube extracted.We can get light microscopic and electronic microscopic pathologic examination through liver tissue and brain tissue.Result: All 8 porcines died in 16h after the diffluence. High serum ammonia,increases of hepatic enzymes, metabolic disorder of blood suger,and the blood coagulation fit the manifestation of acute liver failure.Some porcinesmanifested with lethargy, dysphoria, vomiting and like hepatic coma.The pathology suggested massive necrosis of the liver.The edema ang necrosis were also observed in the brain tissue.Conclusion: This model is fit for the demands of the experiment with a character of fast,conveniece,easy to control.The model is a success.The pathogenesis and the reason for death of the acute liver failure are complicated.The cytokine and the inflammatory medium from the necrotic hepatic cell can lead to a cascade of reaction which results in the multiorganic failure of the landrace model.The Cerebral edema and the brain cell necrosis (brain hernia of the brain stem in some severe cases)are the main fatal cause of the acute liver failure.
