Alkaline burn is of high incidence, severe injure and bad prognosis in chemical bum of eye. Its pathological feature is the infiltration of white blood cells, with a series of tissue damages. From its onset and histological changes, the etiology cannot be the direct damages of alkaline, the severe nutrients insufficiency after alkaline burn cause the decrease in the quality and quantity of collagen synthesis, activate the release of collagenase , accelerate the occur of cornea ulcer. In a word, the pathogenesis of alkaline is complicated. The reconstruction of complete and health cornea epithelium after alkaline burn is one of the keys of treatment. The former Soviet scholar Makapob. n. r has used diaphragm dialysis to treat acute bacterial keratitis. The diaphragm has provided a biomembrane cornea contact lens for the patients, it protects the cornea epithelium to proliferate and grow under it. It is not only bandage for the wound, also the medicine transfer facility.Materials1. Animals: white rabbit weigh 2.0 -2.5Kg.2. Materials; imported teeth plastic powder, human fetus amniot-ic mebrane, 100% glycerol, glucose solution, vitamin C injection, gentamycine injection, 2% fluorescence.3. Instruments: Olympus light microscope, electronic microscope, blood sugar meter, refrigerator.Methods1. Making of diaphragm dialysis apparatus:(1) Making of the dialysis supporter;(2) Making of dialysis diaphragm;(3) Making of dialysis solution;2. Making of alkaline burn animal model;8mm filtrating paper soaked with 2N NaOH solution was put on the rabbit cornea for 1 minute.3. Diaphragm dialysis treatment for alkaline bum; Experimental group 15 rabbits; diaphragm dialysis 1 h our, 2times per day for 3 days; diaphragm dialysis 1 hour , 1 time per day for 12 days;control group 1, 15 rabbits; eye drop of dialysis solution, four times per day;control group 2,16 rabbits; amniotic membrane transplantation, 4 hours after alkaline burn.4. Morphological observation;Fluorescing the cornea every day after the alkaline burn to observe the cornea healing. Kill 5 rabbits at 3days, 5 days, 7 days and 15 days; remove the cornea to study under the light microscope and e-lectronic microscope.5. AnalysisBefore killing the rabbit, 0. 1 - 0. 2ml aqueous humor was removed to measure the glucose level. Perform the student's t test, p <0.05 was thought of significant difference.Result1. Morphology(1) Cornea staining: there was no difference between the groups in the first 3 days of alkaline burn. In 5 - 7 days, spot and linear staining area were seen in the experimental group, and thicker staining area in the control group. In 12 -15 days, no cornea staining or only spot was seen in the experimental group, while in the control group, the ulcer was deeply stained.(2) Light microscope findings: In 3 days, only 1-2 layers of epithelium was seen in both group. In 7 days ,2-3 layers epithelium found in the experimental group, the collagen fiber slightly deformed; in the control group of eye drop, epithelium defect and collagen fiber break was found; in the amniotic membrane transplantation group, vesicular degeneration was found in the epithelium, there was space between the epithelium and the stoma. In 15 days ,4-5 layers of epithelial cells was found, and the stoma was proliferated, no neovascu-larization; in the control groups, epithelium defect and WBC infiltration in stroma and neovascularization were found.( 3 ) Electronic microscope findings: at 3 days, in the experimental group, the reticulum were edema in the cell plasma, in the control group, the cellular structure disappeared; at 7 days, in the experimental group, 2-3 layers of flat epithelial cells were seen, and the cell conjuncture was loose, in the endothelium, cell rich in plasma and vascular cells were both seen.
