BackgroundMelatonin is a kind of hormone secreted by pineal gland,which can modulate the axle of hypothalamus and immunity,affect the secretion of hormone,get rid of free radical and so on. The melatonin is used widely.The recent studies revealed that the melatonin level in the body of acute cerebral infarction patients was low and the ratio of T lymphocyte subpopulation in peripheral blood was disturbance.The numbers of CD3+ and CD4+ T lymphocyte cells in those patients were low and the value of CD4+/CD8+ was decreased. In the anticancer studies,the melatonin can activate the anticancer immunity and prolong the life of the patients suffering from tumor. The relative studies also prove that melatonin can modulates the immunity through the axle of hyposalomus-pypophysis-supra. So it is pratical important to study the intervention of melatonin in the T lymphocyte subpopulation in peripheral blood of patients with acute cerebral infarction for preventing varies kinds of infective complications in clinic. ObjectiveThough the observing of the influence of melatonin on the T lymphocyte subpopulation in peripheral blood of patients with acute cerebral infarction,to explore the restoring methods for the ACI patients. Methods40 cases selected with ACI(acute cerebral infarction) were divided randomly into MT group and vitamin C group. And 20 healthy persons were selected as healthy control group. 9 mg of melatonin and 300 mg of Vitamin C were given respectively once a day from the third day after the onset of ACI. Thenumbers of CD3\ CD4+ and CD8+ T lymphocytes in peripheral blood were examined with flow cytometry (FCM,with direct immunofluroscent staining technique ) on the day before the intervention and on the day of 7 and 14 after that. The numbers of WBC and lymphocytes and the ratio of the occurance of the infective complications were also counted. Results:1. The changes of the T lymphocyte subpopulation before and after the intervention:The numbers of CD3\ CD4\ CD8+ T lymphocyte and the ratio of CD4+/CD8+ in peripheral blood between the melatonin group and the vitamin C group were not significently different. (P>0.05) . To compare with the healthy group,the melatonin group and vitamin C group were signifecently different(P<0.05). The numbers of CD3\ CD4+T lymphocytes and the ratio of CD4+/CD8+ in melatonin group were higher than those in vitamin C group. But the number of CD8+ T lymphocytes in melatonin group was lower than that in vitamin C group.2. The numbers of white blood cell and lymphocytes:The numbers of white blood cells lymphocytes and the percentage of lymphocytes in melatonin group had not been significently different from those in vitamin C group befor the melatonin was given. But to compare with those in the healthy group,the difference was significent (P<0.01) .On the seventh day and the forteenth day,the number of white blood cells in peripheral blood in melatonin group was decreased,while the number of lymphocytes and the percentage were increased. But to compare with those in the vitamin C group,the difference was significent (P<0.01) .3. The infective complications in the ACI patients after the intervention:The infection of upper respiratory tract and pneumonia:3 cases in melatonin group and 10 in vitamin C group;the infection of urinary system:0 in melatonin group and 2 in vitamin C group;the totle rate of infection:15% in melatonin group and 55% in vitamin C group;The difference of the whole infective rate between the MT group and the vitamin C group was significant (x2=7.03,P<0.01) . 4. The neurological scores of CSS in patients with cerebral infarction:The scores of CSS in melatonin group and vitamin C group were respectively as 23.30+4.87 and 23.45+4.14 before intervation,while they were 15.60+5.54 and 19.95+4.43 on the 14th day after intervation. And their difference were signigicent (P<0.01) . Conclusion:1. Melatonin can relief the suppressed condition of the T lymphocyte after cerebral infarction to some extent.
