IntroductionAcute pancreatitis ( AP) is a disease with wide clinical variation, which makes its diagnosis complex. Serum / urinary amylase measurement is a standard diagnostic method, although it was shown to be unable to recognize one fifth of AP patients . There is an urgent need to improve the early recognition of patients with severe AP, especially those with subsequent organ failure, so that they can be sent at an early stage to a centre with facilities for maximal intensive care and specialists in endoscopes, radiological and surgical management of AP patients. Procalcitonin (PCT) is a 116 amino acid protein with a sequence identical to that of the prohormone of calcitonin (32 amino acids). PCT (Procalcitonin) is indicated for use primarily as a diagnostic parameter for bacterial infections triggering a systemic - inflammatory reaction in the body ( sepsis, septic shock). Local limited bacterial or organ - related infections and capsulated abscesses induce a slight increase in PCT if at all. We want detect that if the Procalcitonin can be used in strip the severe AP cases from the edematous pancreatitis cases.Material and methods1 Material1.1 40 male wistar Rats1.2 pentobarbital1.3 5% and 3% sodium taurocholate1.4 EDTA anticoagulated tube1. 5 PCT ) - Q test ( B. Inject 5% or 3% sodium taurocholate with speed of 6ml/h, last 3 -5mins.3 Samples12h after injection take 2ml blood from heart for pet and amylase test and remove the pancreas which sent for pathologic test4 Sample testthe results of pet and amylase provide by laboratory centre and pancreatic pathologic check perform by pathologic departmentResultsThe SAP group has two rats die in 12h, and all of the rats in edematous pancreatitis group are live when taking the sample. 15 rats in SAP group are detected having pancreas necrosis. All rats in edematous pancreatitis group are found with mild pancreatitis. The values of PCT were higher obviously in rats with severe AP than in those with edematous AP. And the serum amylase between two group have no significant difference in statistics.DiscussionAcute pancreatitis ( AP) is a disease with wide clinical variation, which makes its diagnosis complex. Most of the cases are mild and conservative treatment results in a rapid recovery in most of them. However, severe AP constitutes 15 -20% of all cases. In recent decades, mortality rate of severe AP has decreased to 15 -20% . Severe AP is now recognized to be a two - phase systemic disease. In the first phase, extensive pancreatic inflammation and/or necrosis are followed by a systemic inflammatory response syndrome (SIRS) that may lead to multiple organ dysfunction syndromes ( MODS) within the first week. About 50% of deaths occur within the first week of the attack, mostly from MODS, which is not different from the systemic complications found in other diseases or injuries ( e. g. , sepsis, major trauma, bums). Unless the first phase is arrested and reversed by natural defenses or therapeutic intervention, the second phase ensues usually after the second week of onset, and includes the formation of infected pancreatic necrosis or fluid collection with possible progression to overt sepsis, MODS and death .
